Die Spitex Geborgene Hände GmbH begleitet Menschen professionell, respektvoll und herzlich in ihrem eigenen Zuhause. Zur Verstärkung unseres Teams suchen wir eine diplomierte Pflegefachperson HF/FH, die pflegefachliches Arbeiten gerne mit Abklärungen, Planung und administrativen Aufgaben verbindet.

Der Schwerpunkt deiner Tätigkeit liegt auf pflegerischen Abklärungen und Assessments, Pflegeplanung, der fachlichen Begleitung pflegender Angehöriger sowie den dazugehörigen administrativen Aufgaben. Je nach Erfahrung und Interesse übernimmst du die Fallführung.

Bei uns erwartet dich ein überschaubares, engagiertes Team mit klaren Strukturen und kurzen Entscheidungswegen. Du arbeitest zu festen Büro\- und Einsatzzeiten tagsüber von Montag bis ohne \- und Wochenenddienste.

Unser Büro befindet sich zentral in Zürich\-Oerlikon, wenige Minuten vom Bahnhof entfernt und mit dem öffentlichen Verkehr sehr gut erreichbar.

Deine Aufgaben

  • Durchführung pflegerischer Abklärungen
  • Erstellung, Anpassung und Evaluation der Pflegeplanung
  • Fachliche Einschätzung und sorgfältige Dokumentation der Pflegesituation
  • Erledigung administrativer Aufgaben rund um den Pflegeprozess
  • Beratung und Unterstützung von Klientinnen, Klienten und Angehörigen
  • Je nach vereinbartem Aufgabenbereich Übernahme der Fallführung
  • Unterstützung bei komplexen medizinischen Situationen

Dein Profil

  • Abgeschlossene Ausbildung als diplomierte Pflegefachperson HF oder FH
  • Erfahrung in der ambulanten Pflege ist von Vorteil.
  • Selbstständige, strukturierte und verantwortungsbewusste Arbeitsweise
  • Einfühlungsvermögen und Freude an der Arbeit mit Menschen
  • Gute Deutschkenntnisse in Wort und Schrift
  • Aufgrund unseres Klientenstamms sind Sprachkenntnisse in Albanisch besonders erwünscht.
  • Führerausweis Kategorie B.
