Il comprend l’ensemble des services aux utilisateurs, de l’admission au départ des résidents, en veillant à créer, maintenir et développer la qualité de vie des personnes accueillies dans toutes leurs activités quotidiennes hors professionnelles en veillant à leur proposer des lieux de vie adaptés aux modes de vie actuels, en leur offrant des soins médicaux et de confort, en leur proposant des activités quotidiennes, individuelles et/ou collectives, des animations et des événements réguliers ou ponctuels. nous recherchons des : éducateurs/trices référent/es 80% IMPORTANT : seules les candidatures soumises via le site internet seront prises en compte. Le rôle et les responsabilités Motivé/e à : Accompagner les personnes dans la gestion de leur vie quotidienne et les soutenir dans leur parcours de socialisation. Participer à des activités éducatives de groupe ou individuelles, en tenant compte des besoins exprimés et des objectifs fixés. Contribuer à l’encadrement, au soutien et à la stimulation des personnes en difficulté. Favoriser le lien entre les personnes accompagnées, les équipes éducatives et les autres intervenants. Participer aux réunions d’équipe et collaborer avec les collègues dans une logique pluridisciplinaire. Assurer des tâches administratives simples en lien direct avec le suivi éducatif (observations, transmissions, comptes rendus). Respecter les règles d’hygiène, de sécurité et de confidentialité propres au cadre institutionnel. Être l’interlocuteur principal pour un ou plusieurs bénéficiaires et leur famille. Élaborer, suivre et évaluer le PPI en coordination avec l’équipe pluridisciplinaire. Assurer la cohérence des interventions éducatives autour du bénéficiaire. Rédiger les documents de suivi et transmettre les informations pertinentes à la direction et aux partenaires concernés. Le Profil Vous êtes titulaire d’un diplôme reconnu en éducation sociale (HES, ES). Certification désirée : idéalement PF. Vous avez acquis de l’expérience, 5 ans dans ce domaine et êtes intéressé/e par le handicap. La rigueur, l’enthousiasme et le sens des responsabilités font partie de vos qualités professionnelles. Vous êtes à l’aise avec les outils informatiques usuels et possédez un permis de conduire. Nous avons suscité votre intérêt ?
