Ihre Aufgaben – Präventiv, langfristig, persönlich

Eigenverantwortliche Durchführung von Gesundheitsuntersuchungen, Vorsorgechecks und Risikoprofilanalysen

Betreuung von Patientinnen und Patienten mit akuten Symptomen oder chronischen Erkrankungen wie Diabetes, Hypertonie, COPD, Depression etc.

Beratung zur Gesundheitsförderung, Ernährung, Bewegung, Impfstatus und Lebensstilveränderung

Ganzheitliche Behandlungsplanung mit Einbezug sozialer, psychologischer und familiärer Aspekte

Nachbetreuung nach Spitalaustritten oder Operationen, inklusive Medikamentenmanagement und Therapieplanung

Interpretation und Besprechung von Laborwerten, EKG, Röntgen- und Ultraschallbefunden

Zusammenarbeit mit Spezialisten, Physiotherapie, Pflegeeinrichtungen und Sozialdiensten

Ihr Profil – Vertrauensvoll, verlässlich, strukturiert

Facharzttitel Allgemeinmedizin FMH oder gleichwertige MEBEKO-Anerkennung

Eintrag im MedReg des Kantons Zürich

Freude an der kontinuierlichen Betreuung von Patientinnen und Patienten

Fähigkeit, medizinisch komplexe Zusammenhänge verständlich zu vermitteln

Eigenständige, lösungsorientierte und teamfreundliche Persönlichkeit

Sehr gute Deutschkenntnisse, jede weitere Sprache ist ein Plus
