Deine Qualifikationen

  • Dipl. Pflegefachperson FH (Bachelor) mit CAS in Palliative Care oder Dipl. Pflegefachfrau HF mit MAS im Bereich Palliative Care
  • Mehrjährige Berufserfahrung im Bereich Palliative Care idealerweise im ambulanten Bereich
  • Kompetente, selbständige Persönlichkeit mit hohem Verantwortungsbewusstsein
  • Gute EDV\-Kenntnisse
  • Führerausweis der Kategorie B

Dein Fachgebiet

  • Du übernimmst die fachliche Führung im Team Palliative Care und unterstützt die Leitung Pflegeentwicklung in strategischen Entscheiden zum Fachbereich.
  • Du entwickelst unser Angebot in Palliative Care systematisch und professionell gemäss Aargauer Palliative Care Konzept weiter.
  • Du hast den Überblick über fachlich komplexe Situationen im Bereich und unterstützt gemeinsam mit dem Team Palliative Care die Pflegeteams in der Umsetzung einer professionellen palliativen Versorgung unserer Klientinnen und Klienten.
  • Du bietest für das Team Palliative Care einen starken fachlichen Support, übernimmst pflegerische Leistungen bei den Klientinnen und Klienten vor Ort und nutzt Bedside Coaching als Instrument zur Befähigung der Mitarbeitenden.
  • Du agierst intra\- und interdisziplinär, vernetzend und unterstützend.
