Ihre Aufgaben

- Individuelle Pflege und Betreuung von Patienten im häuslichen Umfeld

- Unterstützung bei der Körperpflege, Medikamentenverabreichung und Mobilisation

- Förderung der Selbstständigkeit der Patienten durch gezielte Maßnahmen

- Enge Zusammenarbeit mit Angehörigen und anderen Fachpersonen

- Dokumentation der durchgeführten Pflegeleistungen

Ihr Profil

- Abgeschlossene Ausbildung als Fachmann / Fachfrau Gesundheit EFZ

- Erfahrung in der ambulanten Pflege oder Interesse daran

- Freude an einer selbstständigen und mobilen Tätigkeit

- Belastbarkeit und Verantwortungsbewusstsein

- Führerschein von Vorteil
