DAHEIM ist ein gemeinnütziger Verein mit Sitz in Zürich. Seit 1995 bieten wir Menschen mit psychischen und somatischen Beeinträchtigungen einen Ort, an dem sie zur Ruhe kommen und ihre Ressourcen wiederentdecken können. In unseren familiären Wohngruppen in Zürich (Seebach, Höngg, Altstetten) leben wir das Prinzip: -So viel Betreuung wie nötig, so viel Selbstständigkeit wie möglich-.

Bei uns erwartet dich ein kleines, engagiertes Team, das auf gegenseitige Wertschätzung, offene Kommunikation und eine klare Haltung setzt: Menschen mit Unterstützungsbedarf sind Expert:innen ihres eigenen Lebens - wir begleiten, nicht bevormunden.

Deine Aufgaben

  • Bezugspersonenarbeit: Beratung und Förderung sowie Aktivierung vorhandener Ressourcen unserer Bewohner:innen im teilstationären Setting
  • Interdisziplinäre, professionelle Zusammenarbeit mit Fachpersonen, Angehörigen und Amtsstellen
  • Routinierte Kriseninterventionen
  • Verlaufsdokumentation führen und wichtige Entwicklungen, Ziele und Vereinbarungen festhalten sowie Berichte schreiben
  • Tages- und Abenddienst - keine Nachtdienste, keine Pflege im medizinischen Sinne
  • Teilnahme an regelmässigen Team- und Fallbesprechungen und proaktive Umsetzung und Weiterentwicklung unseres Betriebskonzepts
  • Organisation und Durchführung von Einzel- oder Gruppenaktivitäten, Festen und Ausflügen (nach Absprache)
  • Enge Zusammenarbeit mit der Leitung Betreuung
  • Pikettbereitschaftsdienst nach Absprache

Was du mitbringst

  • Abgeschlossene Ausbildung als Fachperson Betreuung
  • Mindestens drei Jahren Berufserfahrung im sozial-psychiatrischen Setting oder betreuten Wohnen
  • Strukturierte Arbeitsweise und ein hohes Mass an Eigenverantwortung sowie Organisationsgeschick
  • Hohe Flexibilität und Belastbarkeit
  • In Besitz von Führerausweis Kat. B
  • Gute Deutschkenntnisse in Wort und Schrift
  • Gute PC Kenntnisse
