Dem Seewadel \- Zentrum für Gesundheit und Alter ist es eine Herzensangelegenheit, Menschen jeden Alters zu ermöglichen, in ihren vertrauten vier Wänden zu bleiben. Unsere umfassenden Dienstleistungen zielen darauf ab, die Selbständigkeit zu fördern und gleichzeitig Familienangehörigen und nahestehenden Personen Unterstützung zu bieten. Mit unserem engagierten und hochmotivierten Spitex\-Team stehen wir für herzliche Betreuungsdienste und eine begleitende ambulante Pflege. Wir suchen dich, zur Verstärkung unseres Spitex\-Teams nach Vereinbarung: Fachperson Gesundheit EFZ Spitex (40 \- 80%)Ihre Kernaufgaben • Professionelle Pflege und Betreuung der Klientinnen und Klienten im häuslichen Umfeld • Zusammenarbeit mit den fallführenden Pflegefachpersonen sowie weiteren internen und externen Partnern • Lückenlose elektronische Dokumentation der Pflegeleistungen • Mitverantwortung für die stundenweise Planung und Koordination der Pflegeeinsätze an mehreren Tagen pro Woche – ergänzend zur direkten Pflegearbeit • Aktive Mitgestaltung einer modernen und zukunftsorientierten Spitex Ihre Kompetenzen • Abgeschlossene Ausbildung als Fachperson Gesundheit EFZ • Selbstständige, verantwortungsbewusste und organisierte Arbeitsweise • Freude an der Einsatzplanung sowie ausgeprägte Koordinations\- und Organisationsfähigkeit. • Teamfähigkeit, Kommunikationsstärke und Einfühlungsvermögen im Umgang mit Klientinnen und Klienten sowie Angehörigen • Sicherer Umgang mit digitalen Arbeitsmitteln sowie Mobilität (Führerausweis Kat.
