Ihre Aufgaben

  • Fachliche Führung, Unterstützung und Befähigung des Teams in komplexen Pflegesituationen
  • Hauptverantwortung für die Pflegebedarfserfassung mit RAI LTCF
  • Prozessverantwortung für das Ein- und Austrittsmanagement
  • Klinische Situationen mittels Assessments einschätzen und adäquate Massnahmen ableiten
  • Förderung und Sicherstellung evidenzbasierter Pflegeinterventionen
  • Pflegequalität anhand der medizinischen Qualitätsindikatoren evaluieren, Massnahmen ableiten und Verbesserungen einführen
  • Fallbesprechungen planen und durchführen

 

Ihr Profil

  • Abgeschlossenes Studium als Pflegefachperson HF / FH
  • Supervisor RAI LTCF oder Interesse an der Absolvierung
  • Berufserfahrung in der Langzeitpflege
  • Interesse an interprofessioneller Zusammenarbeit
  • Klare Kommunikation und Durchsetzungsvermögen
