Und dies in einem Arbeitsmodell, das in der Pflege nicht gerade zum Standard gehört.

Für eine etablierte Organisation im ambulanten Pflegebereich in der Region Basel suchen wir per sofort oder nach Vereinbarung eine erfahrene

Fallführende Pflegefachperson HF/FH 80-100% - Raus aus dem Schichtkarussell. Rein in die Fallführung... #

Ihre Aufgabe:

Sie begleiten Menschen, die zu Hause gepflegt werden, sowie deren Angehörige. Dabei sind Sie nicht einfach ausführende Pflegefachperson, sondern fachliche Ansprechperson, Berater und Koordinator.

Sie

  • klären Pflegesituationen und den individuellen Bedarf ab
  • erstellen und überprüfen Pflegeplanungen
  • beraten, instruieren und begleiten Angehörige
  • stellen die fachliche Qualität der Pflege sicher
  • dokumentieren sauber und zeitnah
  • koordinieren bei Bedarf mit Ärzten und weiteren involvierten Stellen
  • übernehmen einen wesentlichen administrativen Anteil
  • leisten punktuell praktische Pflege- und Pikettdienste

Der Schwerpunkt liegt klar auf Fallführung, Beratung, Coaching und Organisation. Nicht auf klassischer Schichtarbeit am Bett.

Das bringen Sie mit:

  • Sie verfügen über ein in der Schweiz anerkanntes Pflegediplom HF/FH/DNII/AKP und bringen mehrere Jahre Berufserfahrung mit
  • Idealerweise kennen Sie die ambulante Pflege bereits aus eigener Erfahrung. Spitex-Praxis ist deshalb ein grosses Plus

Ebenso wichtig sind:

  • selbstständige und strukturierte Arbeitsweise
  • gute kommunikative Fähigkeiten
  • Freude an Beratung und Begleitung
  • Sicherheit in Administration und Dokumentation
  • organisatorisches Geschick und gesunder Pragmatismus
  • Führerausweis Kategorie B
  • gute IT-Anwenderkenntnisse
  • Französischkenntnisse wären ein zusätzlicher Trumpf
  • Und ja: Lebenserfahrung schadet in dieser Funktion ganz sicher nicht.

Was diese Stelle besonders macht

Hier verbinden sich Pflegewissen, Beratung, Aussendienst und Administration. Die Tätigkeit findet grundsätzlich von Montag bis Freitag statt. Das bringt einen Arbeitsrhythmus mit sich, den viele Pflegefachpersonen nach Jahren im Schichtbetrieb durchaus zu schätzen wissen. Ganz 'weg vom Bett' ist die Stelle nicht. Aber deutlich weiter weg vom klassischen Pflegealltag als viele andere Funktionen. Vielleicht möchten Sie die Pflege gar nicht verlassen. Vielleicht möchten Sie einfach nur anders darin arbeiten. Dann sollten wir miteinander reden.
