Ihre Aufgaben ##

  • Fallführende Betreuung und Beratung der Patientinnen und Patienten im akutpsychiatrischen «Home Treatment».
  • Ihre Aufgaben als Fallführung sind u.a. regelmässige Hausbesuche, Behandlungsplanung und regelmässige Behandlungsüberprüfung im Kernteam, Planung von Systemgesprächen, Anamneseerhebung, gemeinsame Erstellung der Austrittsberichte
  • Durchführung von pflegerischen Aufgaben (Blutentnahmen, EKG, Medikamentenmanagement, Vitalzeichenkontrollen, Injektionen) und patientenbezogenen administrativen Arbeiten im Schichtdienst (Früh- und Spätdienst)
  • Übernahme von Pikettdiensten
  • Aktive Zusammenarbeit im Team, mit externen Institutionen und dem Umfeld der Patientinnen und Patienten

Ihr Profil ##

  • Ausbildung als dipl. Pflegefachfrau / -mann HF
  • Mindestens zwei Jahre Erfahrung in der psychiatrischen Pflege
  • Führerausweis (Kat. B)
  • Versierte Sprach- und PC-Kenntnisse
  • Offene, flexible und selbständige Persönlichkeit mit hoher Sozialkompetenz
