Les Peupliers assure 7j/7j auprès des 7 CMS de la Fondation soins Lausanne des remplacements de jour, de soir comme de nuit afin de garantir la continuité des soins de nos clients.

Afin de compléter l'équipe du CMS des Peupliers, La Fondation Soins Lausanne recherche pour le de suite ou à convenir :

Infirmier* *coordinateur (H/F)

Taux d'activité de 80%

Contrat fixe à durée indeterminée

Vos missions

  • Identifier, analyser les besoins immédiats du client et établir le premier plan d'intervention afin d'optimiser la continuité des soins.
  • Sécuriser le retour à domicile des clients et coordonner les soins durant la prise en charge.
  • Dispenser des soins durant le temps de prise en charge.
  • Assurer un service de garde téléphonique les soirs, week\-end et jours fériés.
  • Collaborer avec les intervenant du réseau client et répondre à leurs demandes.
  • Intervenir au besoin sur des situations aiguës à domicile.

Votre profil et vos compétences

  • Bachelor en soins infirmiers ou titre jugé équivalent (diplôme étranger reconnu par la CRS)
  • Expérience dans le domaine médico\-social et dans la collaboration interdisciplinaire
  • Connaissance dans les réseaux de soins du canton de Vaud
  • Sens des responsabilités, des priorités et autonomie
  • Permis de conduire indispensable et véhicule souhaitable
