* La Résidence des Diablerets est un établissement médico\-social, dont la mission est de type gériatrique et psycho\-gériatrique compatible. D’une capacité d’accueil de 22 lits, elle héberge des résidents âgés en longs et courts séjours et dispose d’un centre d’accueil temporaire.

Votre profil :

  • Intérêt pour la personne âgée : Intérêt marqué pour la gériatrie et la psychogériatrie ; une première expérience en EMS constitue un atout
  • Aisance relationnelle \& Posture : Caractère calme, apaisant, capacité d'écoute, de recul et sens aigu des responsabilités.
  • Organisation et leadership : Capacité à prioriser, coordonner les soins, accompagner l’équipe et prendre des décisions adaptées aux situations cliniques.
  • Esprit d’équipe : Aisance dans la collaboration interdisciplinaire et dans les relations avec les résidents, les proches et les partenaires.
  • Compétences numériques : Maîtrise des outils informatiques institutionnels et aisance dans la tenue du dossier informatisé du résident (DIR/Carefolio).
  • Bachelor HES\-SO en Soins infirmiers exigé (ou équivalence enregistrée auprès de la Croix\- suisse).
  • Formations complémentaires (un atout) : Titulaire d'un CAS (Praticien·ne Formateur·trice, Psycho\-gériatrie, Soins palliatifs) ou formation HPCI.

Vos tâches principales :

  • Organiser et garantir la qualité des soins : Évaluer les besoins, planifier, dispenser et superviser les soins infirmiers, en assurant leur qualité, leur sécurité et leur continuité.
  • Organiser et coordonner les activités de soins : Définir les priorités, coordonner l’organisation quotidienne des soins et assurer une prise en charge adaptée à l’évolution de l’état de santé des résidents.
  • Encadrer et soutenir l’équipe soignante : Guider et superviser les ASSC, auxiliaires de santé, étudiant·es et stagiaires dans leurs activités quotidiennes.
  • Coordonner la prise en charge et collaborer en réseau : Assurer le lien avec l’équipe interdisciplinaire, les médecins traitants, les familles et les partenaires externes.
  • Garantir la traçabilité et le suivi clinique : Assurer la qualité des transmissions, la mise à jour des démarches de soins et la tenue rigoureuse du dossier informatisé du résident (Carefolio)
