Infirmier·ère en soins à domicile Infirmier·ère en soins à domicile ==================================== \\Parce que chaque intervention à domicile est unique, nous recherchons des infirmier·ères autonomes, organisé·es et doté·es d'un excellent sens relationnel pour accompagner les patients dans leur environnement de vie.\\ Pour plusieurs de nos partenaires actifs dans les soins à domicile à travers le \\canton de Vaud\\, nous recrutons des \\infirmier·ères\\ pour des \\missions temporaires de courte et longue durée\\. Les interventions sont réparties par secteur géographique selon votre lieu de résidence et les besoins des équipes. Infirmier·ère en soins à domicile ==================================== Votre mission ------------- Au sein d'équipes pluridisciplinaires, vous assurez la prise en charge globale des patients à domicile dans le respect de leur projet de soins, en favorisant leur autonomie et leur maintien à domicile. Vos principales responsabilités : \* Réaliser les soins infirmiers techniques et relationnels auprès des patients. \* Évaluer l'état de santé des bénéficiaires et assurer un suivi adapté. \* Participer à l'élaboration et à l'actualisation du projet de soins. \* Collaborer avec les médecins, les proches aidants et les différents intervenants du réseau de santé. \* Assurer les transmissions et la traçabilité des prestations dispensées. \* Contribuer à la prévention, à l'éducation thérapeutique et au maintien de l'autonomie des patients. \* Participer à la continuité et à la qualité des soins à domicile. Votre profil ------------ \* Bachelor HES\\-SO en soins infirmiers ou diplôme étranger reconnu par la Croix\\-Rouge Suisse. \* Expérience professionnelle en soins à domicile, en médecine, en gériatrie ou en milieu hospitalier, un atout. \* Capacité à travailler de ère autonome tout en s'intégrant dans une équipe pluridisciplinaire. \* Excellentes compétences relationnelles et sens de l'organisation. \* Flexibilité, adaptabilité et gestion efficace des priorités. \* Permis de conduire indispensable. \* Véhicule personnel, un atout selon les régions d'intervention.
