Pour nos CMS, nous recherchons : Infirmier\-ère référent\-e en santé mentale à 80\-100% CMS Yverdon (Est et Ouest) : 80\-100% Merci de préciser dans votre lettre de motivation pour quel CMS vous postulez. (1 poste/cms) Votre mission En qualité d'infirmier\-ère en santé mentale, votre rôle en qualité de personne ressource pour votre future équipe implique une maîtrise avancée de compétences spécifiques : Évaluation clinique : Réaliser des évaluations régulières pour adapter les soins aux besoins évolutifs et complexes des clients. Collaboration interprofessionnelle : Travailler en synergie avec un réseau de soins multidisciplinaire pour optimiser la prise en charge. Leadership et mentorat : Fournir guidance et support à vos collègues, grâce à votre rôle de personne ressource en santé mentale, agissant comme référence au sein de votre équipe. Planification, coordination et gestion de la documentation : Organiser des plans de soins personnalisés en partenariat avec les clients tout en maintenant à jour les dossiers clients conformément aux protocoles, y compris les fiches OPAS. Votre profil Vous détenez un diplôme HES d'infirmier\-ère ou un titre reconnu comme équivalent, complété par au moins deux années d'expérience en psychiatrie adulte et/ou gériatrique. Un diplôme post\-grade en santé mentale constituerait un plus. Doté(e) d'autonomie, de polyvalence et d'une grande capacité d'écoute, vous avez un intérêt marqué pour le maintien à domicile et le travail collaboratif avec différentes disciplines. Vous avez la capacité de vous adapter facilement à diverses situations, et vous excellez dans la communication et la négociation. Vous possédez un permis de conduire et, idéalement, un véhicule personnel.
