Wir suchen dich! Zur Unterstützung unserer Teams suchen wir ab sofort oder nach Vereinbarung: Pflegefachperson HF/FH 70-90% Standort Thalwil Deine Aufgaben Bedarfsabklärungen nach interRAI-HC Selbständige, eigenverantwortliche Pflege und Betreuung der Kunden Übernahme der Fallführung Erheben der Pflegebedürfnisse und des Pflegeaufwandes Erstellen, Anpassung und Auswertung der Pflegedokumentation nach interRAI-HC Intervention gemäss Pflegeplanung ausführen und dokumentieren Führen von Beratungsgesprächen Laufende Überprüfung der Qualität und Wirksamkeit der Pflege Zusammenarbeit mit Angehörigen, Ärzt:innen, Spitälern, Heimen und anderen Fachstellen Anforderungsprofil eine abgeschlossene Ausbildung (HF/FH) professionelle, selbstständige Arbeitsweise eine breite Berufserfahrung Sozialkompetenz, Belastbarkeit Flexibilität, insbesondere bezüglich Einsatzzeiten gute PC Kenntnisse  Fahrausweis Kat.
