Für eine moderne Institution im Bereich Altersmedizin suchen wir eine engagierte Pflegefachperson HF/FH. Anforderungsprofil ##

  • Abgeschlossene Ausbildung als Pflegefachperson HF/FH
  • Berufserfahrung in der Akutpflege, Rehabilitation oder Geriatrie
  • Selbstständige und verantwortungsbewusste Arbeitsweise
  • Freude an interdisziplinärer Zusammenarbeit
  • Hohe Sozial- und Kommunikationskompetenz
  • Belastbare und engagierte Persönlichkeit
  • Patientenorientiertes Denken und Handeln
  • Wirtschaftliches Verständnis sowie Organisationsgeschick

Aufgaben ##

  • Verantwortung für die Planung, Steuerung und Evaluation des gesamten Pflegeprozesses
  • Ermittlung individueller Pflegebedürfnisse und Definition geeigneter Pflegemassnahmen
  • Fachliche Begleitung von Patientinnen und Patienten in komplexen Pflegesituationen
  • Beratung und Unterstützung von Angehörigen
  • Zusammenarbeit mit Fachpersonen verschiedener Berufsgruppen
  • Sicherstellung und Umsetzung von Qualitätsstandards und internen Richtlinien
  • Mitwirkung an Optimierungs- und Entwicklungsprozessen im Pflegealltag
  • Unterstützung bei der Begleitung von Lernenden und Studierenden
  • Förderung einer patientenorientierten und professionellen Versorgung
