Für eine Institution im Bereich der ambulanten Pflege in der Region Basel suchen wir eine diplomierte Pflegefachperson HF/FH mit fundierter Erfahrung in der Psychiatrie. Sie begleiten Menschen mit psychiatrischen und psychosozialen Herausforderungen direkt in ihrem persönlichen Lebensumfeld und unterstützen sie dabei, Stabilität, Selbstständigkeit und Lebensqualität zu erhalten oder weiterzuentwickeln. Die Position bietet Ihnen ein hohes Mass an Eigenverantwortung und Gestaltungsspielraum. Gleichzeitig profitieren Sie vom fachlichen Austausch innerhalb eines spezialisierten Teams. Die Arbeitszeiten sind von Montag bis Freitag vorgesehen, Wochenenddienste fallen nicht an.

Anforderungsprofil ##

  • Abgeschlossene Ausbildung als Pflegefachperson HF/FH oder gleichwertige, SRK-anerkannte Ausbildung
  • Mindestens zwei Jahre Berufserfahrung in der psychiatrischen Pflege bei einem 100%-Pensum beziehungsweise entsprechende Erfahrung bei Teilzeitarbeit
  • Erfahrung in der selbstständigen Steuerung und Gestaltung des Pflegeprozesses
  • Ausgeprägtes Verständnis für komplexe psychiatrische und psychosoziale Situationen
  • Ressourcenorientierte und wertschätzende Haltung im Umgang mit Menschen in unterschiedlichen Lebenslagen
  • Hohes Verantwortungsbewusstsein und Freude an einer selbstständigen Arbeitsweise
  • Gute Kommunikations- und Organisationsfähigkeiten
  • Freude an der interprofessionellen Zusammenarbeit
  • Bereitschaft zur fachlichen und persönlichen Weiterentwicklung
  • Führerausweis Kategorie B und eigenes Fahrzeug

Aufgaben ##

  • Selbstständige Begleitung und Beratung von Menschen in unterschiedlichen psychiatrischen und psychosozialen Situationen
  • Verantwortung für den gesamten Pflegeprozess von der Einschätzung des Unterstützungsbedarfs bis zur Evaluation
  • Professionelle Begleitung bei psychischen Krisen und herausfordernden Lebenssituationen
  • Anwendung geeigneter psychiatrischer Pflegeinterventionen im häuslichen Umfeld
  • Durchführung ärztlich verordneter behandlungspflegerischer Massnahmen
  • Förderung von Selbstständigkeit, Ressourcen und psychischer Stabilität
  • Beratung und Anleitung von Klientinnen und Klienten sowie deren Angehörigen und Bezugspersonen
  • Koordination der Pflege und Zusammenarbeit mit medizinischen, therapeutischen und sozialen Fachpersonen
  • Durchführung und Mitgestaltung von Fallbesprechungen
  • Fachliche Unterstützung von Mitarbeitenden bei psychiatrischen Fragestellungen
  • Mitwirkung bei Bedarfsabklärungen und fachlichen Assessments
  • Beteiligung an der Weiterentwicklung der Pflegequalität
  • Einsätze von Montag bis Freitag ohne Wochenenddienste
