Für das neu entstehende Projekt der peripartalen Krisenbegleitung bei Meliso suchen wir per Januar 2027 oder nach Vereinbarung eine engagierte Persönlichkeit als

Sie möchten in einem interdisziplinären Team und einer bestehenden Organisation ein neues Angebot aktiv mitgestalten und mitentwickeln?

Dann freuen wir uns auf eine interessierte, engagierte und verantwortungsbewusste Fachperson, die sich mit Engagement in die Umsetzungsphase unseres interdisziplinären Projekts einbringt. 

Die sensible Übergangszeit rund um Schwangerschaft, Geburt und frühe Elternschaft erfordert vielschichtige innere und äussere Anpassungen, die für manche Familien eine emotionale Krise bedeuten. Das Projekt peripartale Krisenbegleitung soll Familien in dieser Übergangszeit ambulant aufsuchend in ihrem häuslichen Umfeld begleiten und unterstützen.

 Ihre Aufgaben

  • Aktive Mitarbeit beim Aufbau und der Umsetzung des neuen Pilotprojekts 
  • Mitentwicklung von Prozessen, Strukturen und Angeboten  
  • Durchführung psychiatrischer Bedarfsabklärungen und Erfassung des individuellen Unterstützungsbedarfs von Familien in peripartalen Krisensituationen in ihrem häuslichen Umfeld gemäss den geltenden kantonalen Vorgaben 
  • Fallführung, Pflegeplanung sowie Begleitung und Unterstützung von Familien in peripartalen Krisensituationen in Rücksprache mit der Fachleitung
  • Case Management und interprofessionelle Zusammenarbeit und Vernetzung mit internen und externen Fachpersonen in Rücksprache mit der Fachleitung
  • Fortlaufende Beobachtung und fachliche Einschätzung des psychischen Zustands sowie des Unterstützungsbedarfs  
  • Bei Bedarf Medikamentenmanagement und Begleitung bei der Umsetzung therapeutischer Massnahmen in Rücksprache mit der Fachleitung und dem interdisziplinären Behandlungsteam 
  • Fachgerechte Dokumentation und Mitwirkung an der Qualitätssicherung  
  • Traumasensibles, bindungsorientiertes Arbeiten mit Familien in Krisensituationen 

 Ihr Profil

  • Ausbildung als diplomierte Pflegefachperson HF oder BSc in Pflege FH, im psychiatrischen Fachbereich
  • Gesamthaft mindestens zwei Jahre Berufserfahrung bei einem Beschäftigungsgrad von 100% im psychiatrischen Fachbereich (Teilzeit entsprechend längere Berufserfahrung nötig)  
  • Erfahrung in der ambulanten und aufsuchenden psychiatrischen Versorgung
  • Erfahrung in der Erfassung, Planung, Durchführung und Evaluation psychiatrischer Spitex-Leistungen auf Grundlage des individuellen Pflege- und Unterstützungsbedarfs in Zusammenarbeit mit einem interdisziplinären Team 
  • Erfahrung in der somatischen Pflege
  • Erfahrung in Case Management und interprofessioneller Zusammenarbeit 
  • Idealerweise Erfahrung in der Begleitung von Familiensystemen in peripartalen Krisen sowie Kenntnisse in den Bereichen Bindungsentwicklung, Schwangerschaft, Geburt und frühe Elternschaft - oder die Bereitschaft sich in diesen Themenfeldern gezielt weiterzubilden
  • Die Bereitschaft sich in einem interdisziplinären Team persönlich und fachlich laufend weiterzuentwickeln 
  • Offene, achtsame, verantwortungsvolle Persönlichkeit
  • Selbstständige, belastbare und teamorientierte Arbeitsweise  
  • Freude an interdisziplinärer Zusammenarbeit und am Aufbau eines neuen Angebotes
  • Führerausweis Kategorie B vorhanden
