MISSION

  • Effectuer des soins infirmiers et soins de base au domicile de personnes présentant un état de dépendance physique, psychique ou un handicap.
  • Intégrer dans sa pratique les concepts de promotion et de prévention de la santé.
  • Mettre en oeuvre les projets de soin des clients, en collaboration avec l'équipe pluridisciplinaire du CMS et le réseau.
  • Assurer le rôle de responsable de situation en effectuant des analyses cliniques et en réalisant et déléguant des prestations de qualité

RATTACHEMENT/LIEU CMS de Prilly Nord

PROFIL

  • Bachelor of Science HES\-SO en Soins infirmiers ou titre jugé équivalent (Reconnaissance Croix\-Rouge pour les diplômes étrangers
  • CAS en soins des plaies ou expérience professionnelle avérée dans le domaine
  • Intérêt pour les soins à domicile et le travail en équipe pluridisciplinaire
  • Aptitude à travailler de manière autonome et à faire face aux situations complexes
  • Excellente analyse clinique et capacité à négocier
  • Très bon sens de l'organisation
  • Permis de conduire (obligatoire)
  • Bonne connaissance des outils informatiques usuels et de la langue française
