Zur Ergänzung unseres Teams suchen wir für die Austritts- und Sozialberatung per 1. Oktober 2026 oder nach Vereinbarung eine:n  

Sozialarbeiter:in 50-80 % ###

Ihre Herausforderung ##

  • Beratung von Patienten unter Einbezug der Angehörigen während dem Spitalaufenthalt zur Vorbereitung des Austritts, Vermittlung von Anschlusslösungen
  • Interdisziplinäre Zusammenarbeit in der Betreuung von schwerkranken und sterbenden Patient:Innen und deren Angehörigen
  • Vermittlung von externen Fachdiensten und medizinischen Hilfsmitteln
  • Beratung von Patienten in versicherungs- und rechtlichen Fragen inkl. Abklärung von Finanzierungsfragen, Erschliessen von finanziellen Unterstützungen oder nicht ausgeschöpften Finanzierungsquellen
  • Einholen von Kostengutsprachen im In- und Ausland
  • Interdisziplinäre Zusammenarbeit mit allen Pflegestationen und dem ärztlichen wie pflegerischen Dienst
  • Zusammenarbeit und Vernetzung mit nachsorgenden Institutionen
  • Mitarbeit in Projekten des Spitals

Ihr Profil ##

  • Abgeschlossenes Studium Soziale Arbeit FH oder gleichwertig
  • Erfahrung im Gesundheitswesen, Sozialdienst, Casemanagement, oder Grundausbildung in der Pflege von Vorteil
  • Sehr gute Kommunikationsfähigkeiten
  • Lösungsorientierter Umgang mit Krisen und Problemsituationen
  • Ausgeprägte Sozialkompetenz und Empathie
  • Bereitschaft interdisziplinär zusammenzuarbeiten
  • Lernfreudig und interessiert Aus-/Weiterbildungen zu absolvieren
  • Professioneller Umgang mit öffentlichen Diensten
  • Motivierte und engagierte Persönlichkeit, die sich durch eine selbstständige Arbeitsweise auszeichnet und Entscheidungen eigenhändig treffen kann
