Sozialarbeiter/-in FH ##

Mit einem Beschäftigungsgrad von 80 % sind Sie in drei Tageskliniken tätig: Der Psychiatrischen Tagesklinik Romanshorn, der Alterstagesklinik Romanshorn und der Alterstagesklinik Weinfelden. Als Sozialarbeiter/-in sind Sie eine wertvolle Ergänzung der Behandlungsteams, bestehend aus Ärzten/-innen, Psychologen/-innen, Pflegefachkräften und Spezialtherapeut/innen. Es erwartet Sie eine abwechslungsreiche, spannende und sinnhafte Tätigkeit.

Sie übernehmen

  • Beurteilung der aktuellen sozialen Situation der Patienten/-innen und Definition der sozialen Problembereiche in Zusammenarbeit mit dem multiprofessionellen Team und den Betroffenen
  • Beratung und Unterstützung der Patienten/-innen und ihrer Angehörigen in Fragen der Arbeit, der Finanzen und des Wohnens sowie von sozialversicherungsrechtlichen Angelegenheiten
  • Kontakte zu Behörden und nachbetreuenden Institutionen halten
  • Mitarbeit im Austrittsmanagement und der Organisation von der weiteren Versorgung und / oder neuen Wohnformen
  • Teilnahme an multiprofessionellen Rapporten, Teamsitzungen und Supervisionen
  • Koordination der sozialarbeiterischen Aufgaben unter Einbezug des gesamten Patientenumfeldes und des multiprofessionellen Teams

Sie bringen mit

  • Abgeschlossenes Studium in Sozialer Arbeit FH 
  • Bei einem im Ausland abgeschlossenen Studium: Anerkennung der Berufsqualifikation in der Schweiz oder die Bereitschaft diese zu erwerben
  • Flexibilität, Selbstständigkeit, Belastbarkeit und Teamfähigkeit
  • Berufserfahrung im psychiatrischen oder medizinischen Kontext ist von Vorteil
  • Gute Kenntnisse des Schweizer Sozialwesens
  • Grundverständnis, Einfühlungsvermögen und Kommunikationskompetenz im Umgang mit psychisch kranken Menschen
