  • Aktive Mitarbeit an Projekt- und Arbeitsgruppen sowie insbesondere bei der Implementierung der Inhouse-Spitex
  • Verantwortung für die Spitex-Bedarfsermittlung mit interRAI HC, Leistungsplanung und-kontrolle sowie Abrechnung
  • Leitung des Pflegedienstes bestehend aus vier Mitarbeitenden in organisatorischer und personeller Hinsicht
  • Aktive Mitwirkung bei der Weiterentwicklung der Pflegequalität
  • Entwicklung und Optimierung von Strukturen, Abläufen und Prozessen im Pflegedienst
  • Unterstützung und Beratung der Wohngruppen und Ateliers bei pflegerischen Fragestellungen
  •  Zusammenarbeit mit der Heimarztpraxis, ausführen der ärztlichen Verordnungen
  •  Aussagekräftige und lückenlose Führung der Verlaufsdokumentation (easyDOK)
  • Übernahme von medizintechnischen Verrichtungen
  • Organisation, Durchführung und Begleitung von medizinischen Terminen
  • Interdisziplinäre Zusammenarbeit mit den Mitarbeiter:innen der Wohngruppen und Ateliers, den gesetzlichen Vertretungen und Angehörigen, sowie weiteren involvierten Stellen

Das bringst du mit: ###

  • Abgeschlossene Ausbildung als dipl. Pflegefachfrau HF/FH/AKP/DNII mit Berufserfahrung aus einem ähnlichen Umfeld, idealerweise ergänzt mit einer betriebswirtschaftlichen Weiterbildung sowie einer Führungsausbildung
  • Erfahrung in der Spitex und mit interRAI HC zwingend sowie in der Umsetzung von Projekten im Pflegebereich, idealerwiese im Themengebiet der Inhouse-Spitex
  • Freude, Neugier und Innovationsgeist für die Ausarbeitung und Implementierung des Angebots Inhouse-Spitex
  • Selbständige und lösungsorientierte Arbeitsweise
  • Aktuelle Kenntnisse in der Akut- und/oder Langzeitpflege
  • Hohe Sozialkompetenz sowie ausgeprägte Kommunikations- und Organisationsfähigkeiten
  • Flexible, belastbare und humorvolle Persönlichkeit mit Freude an der multiprofessionellen Zusammenarbeit
  • Führungserfahrung erwünscht, Führungsweiterbildung? Oder Bereitschaft eine zu absolvieren?
  • Führerausweis Kategorie B und sicherer Umgang mit Microsoft 365 (Word, Excel, Outlook)
