Votre profil

  • Titulaire d'un diplôme d'une école sociale reconnue type HES (filière travail social avec orientation en service social), EESP\-Ecole d'études sociales et pédagogiques Suisse, Bachelor en travail social ou titre jugé équivalent
  • Bonnes connaissances des assurances sociales et du réseau médico\-social vaudois
  • Capacité à s’organiser de manière autonome et facilité d’adaptation
  • Capacités d’écoute et de négociation
  • Permis de conduire indispensable

Votre petit \+

  • Connaissance du domaine des soins à domicile
  • Expérience d’au moins 5 ans dans le domaine médico\-social

Vos futures missions

  • Vous accompagnez les patients et leurs proches pour effectuer des démarches spécifiques et complexes dans un but thérapeutique en s'appuyant sur la relation d'aide et de soutien
  • Vous assurez une activité de liaison et d'orientation avec les proches et le réseau médico\-social
  • Vous êtes partenaire de référence de l'équipe pluridisciplinaire pour les questions sociales
  • Vous assurez un pôle de référence et d'expertise auprès des membres de l'équipe pluridisciplinaire pour les questions ayant trait aux domaines juridique, assécurologique et administratif

Elle intègre également les services logistiques de CMS et l’Espace Prévention La Côte.
