Junior (Trainee) Wmo Consulent

Ben je net klaar met een Wmo-opleiding en wil je jouw eerste stappen zetten als Wmo Consulent? In deze functie doe je praktijkervaring op met het onderzoeken en afhandelen van meldingen binnen de klassieke Wmo. Ook als je nog weinig werkervaring hebt, krijg je de kans om je verder te ontwikkelen binnen het vakgebied.

De focus ligt op hulpvragen rondom hulpmiddelen, voorzieningen, woningaanpassingen, huishoudelijke ondersteuning en vervoer. Je voert onderzoeken uit, stelt (her)indicaties en zorgt voor een zorgvuldige rapportage. Daarbij combineer je kwaliteit met een efficiënte manier van werken.

Wat je gaat doen:

  • Onderzoeken en afhandelen van meldingen binnen de klassieke Wmo;
  • Voeren van gesprekken met inwoners om hun ondersteuningsbehoefte in kaart te brengen;
  • Stellen van indicaties en herindicaties;
  • Behandelen van hulpvragen rondom hulpmiddelen en voorzieningen;
  • Opdoen van ervaring met woningaanpassingen, huishoudelijke ondersteuning en vervoer;
  • Uitvoeren van huisbezoeken wanneer dit voor het onderzoek nodig is;
  • Opstellen en versturen van zorgvuldige rapportages;
  • Inplannen en administratief verwerken van meldingen en aanvragen;
  • Werken met Suite voor het verwerken van gegevens en dossiers;
  • Toepassen van relevante wet- en regelgeving binnen de Wmo;
  • Fungeren als aanspreekpunt voor inwoners en zorgen voor prettig en duidelijk klantcontact;
  • Bijdragen aan het terugdringen van wachttijden met aandacht voor zowel kwaliteit als kwantiteit.

Wat jij meebrengt:

  • Je hebt recent een Wmo-opleiding afgerond;
  • Je hebt 0 tot 12 maanden ervaring met het indiceren van meldingen op het gebied van hulpmiddelen en voorzieningen;
  • Ook wanneer je nog weinig praktijkervaring hebt, nodigen we je uit om te reageren;
  • Je beschikt over hbo werk- en denkniveau;
  • Je hebt kennis van relevante wet- en regelgeving (harde eis);
  • Je beschikt over een geldig rijbewijs B en eigen vervoer voor het uitvoeren van huisbezoeken (harde eis);
  • Je hebt ervaring met Suite;
  • Ervaring met Suite4Zorg / Suite voor het Sociaal Domein is een pré;
  • Je hebt affiniteit met het inplannen van meldingen en administratief verwerken van aanvragen;
  • Je bent digitaal vaardig en kunt zorgvuldig met dossiers en systemen werken;
  • Je vindt contact met inwoners leuk en bent klantgericht;
  • Je bent gemotiveerd om meldingen zorgvuldig en efficiënt af te handelen;
  • Je beschikt over goede communicatieve vaardigheden en kunt zelfstandig werken.
