Wmo Gespreksvoerder Wonen, Rollen en Vervoer

Als Wmo Gespreksvoerder ondersteun je inwoners bij vragen rondom wonen, rollen en vervoer. Je voert keukentafelgesprekken en huisbezoeken, brengt de ondersteuningsvraag zorgvuldig in kaart en kijkt daarbij naar verschillende leefgebieden. Samen met de inwoner onderzoek je wat iemand zelf kan, welke mogelijkheden er binnen het eigen netwerk zijn en welke ondersteuning of voorziening aanvullend nodig is.

Je werkt vanuit het uitgangspunt ‘van zorgen voor, naar zorgen dat’. De uitkomsten van je onderzoek vertaal je naar een duidelijk ondersteuningsplan, gespreksverslag en waar nodig een beschikking. Daarbij werk je nauw samen met zorgaanbieders, leveranciers en andere betrokken partijen.

Wat je gaat doen:

  • Je voert keukentafelgesprekken en huisbezoeken, voornamelijk gericht op wonen, rollen en vervoer;
  • Je onderzoekt en beoordeelt de ondersteuningsvraag van inwoners;
  • Je kijkt integraal naar verschillende leefgebieden en vraagt actief door om de situatie goed in beeld te krijgen;
  • Je onderzoekt samen met de inwoner mogelijkheden vanuit eigen kracht, gebruikelijke hulp, mantelzorg, het sociale netwerk en algemene voorzieningen;
  • Je bepaalt welke ondersteuning of Wmo-voorziening passend is;
  • Je vertaalt de resultaten van huisbezoeken naar ondersteuningsplannen en gespreksverslagen;
  • Je stelt beschikkingen op;
  • Je werkt samen met zorgaanbieders, leveranciers van hulpmiddelen en andere ketenpartners.

Wat jij meebrengt:

  • Je hebt een afgeronde opleiding op minimaal hbo-bachelorniveau (harde eis);
  • Je hebt minimaal 2 jaar aantoonbare werkervaring als Wmo Consulent, Wmo Gespreksvoerder of in een vergelijkbare functie binnen een gemeente (harde eis);
  • Je hebt aantoonbare kennis van en werkervaring met de Wmo 2015 en relevante wet- en regelgeving (harde eis);
  • Je bent in het bezit van rijbewijs B (harde eis);
  • Een afgeronde hbo-opleiding in de richting van Sociaal Pedagogische Hulpverlening, Sociaal Werk of Maatschappelijk Werk en Dienstverlening is een pré;
  • Je hebt ervaring met het voeren van keukentafelgesprekken binnen de Wmo;
  • Je hebt ervaring met het opstellen van ondersteuningsplannen, gespreksverslagen en beschikkingen;
  • Ervaring met iSamenleving is een pré;
  • Je hebt ervaring met voorzieningen op het gebied van wonen, rollen en vervoer;
  • Je kunt effectief samenwerken met zorgaanbieders, leveranciers van hulpmiddelen en andere ketenpartners;
  • Je luistert actief, vraagt goed door en beschikt over empathisch en analytisch vermogen;
  • Je hebt een kritische en objectieve blik en bent in staat zelfstandig tot een zorgvuldig oordeel en besluit te komen.
