Wmo Consulent Klassieke Wmo

Als Wmo Consulent richt je je op het onderzoeken en afhandelen van meldingen binnen de klassieke Wmo. De nadruk ligt op woningaanpassingen, hulpmiddelen, huishoudelijke ondersteuning en vervoer. Je voert zelfstandig onderzoeken uit, stelt (her)indicaties en zorgt voor een duidelijke en zorgvuldige rapportage.

Je werkt binnen een team dat zich richt op het terugdringen van wachttijden. Daarbij is het belangrijk dat je snel zelfstandig inzetbaar bent en kwaliteit weet te combineren met een goede productiviteit. De meldingen worden voor je ingepland, waarna je zelf ruimte hebt om je agenda efficiënt in te richten.

Wat je gaat doen:

  • Zelfstandig onderzoeken en afhandelen van meldingen binnen de klassieke Wmo;
  • Voeren van gesprekken met inwoners om hun ondersteuningsbehoefte zorgvuldig in kaart te brengen;
  • Stellen van indicaties en herindicaties;
  • Beoordelen van hulpvragen rondom woningaanpassingen;
  • Beoordelen van aanvragen op het gebied van hulpmiddelen en vervoersvoorzieningen;
  • Onderzoeken van hulpvragen rondom huishoudelijke ondersteuning;
  • Afhandelen van onderzoeken en opstellen en versturen van rapportages;
  • Uitvoeren van huisbezoeken wanneer dit voor het onderzoek noodzakelijk is;
  • Zorgvuldig toepassen van relevante wet- en regelgeving;
  • Wekelijks afhandelen van gemiddeld 8 tot 10 meldingen;
  • Bijdragen aan het verkorten van de wachttijden, met behoud van kwaliteit;
  • Zelfstandig plannen en organiseren van je werkzaamheden en afspraken.

Wat jij meebrengt:

  • Hbo werk- en denkniveau;
  • Aantoonbare ervaring met het stellen van (her)indicaties binnen de klassieke Wmo;
  • Minimaal 3 tot 24 maanden ervaring met het indiceren van meldingen op het gebied van begeleiding en dagbesteding;
  • Goede kennis van relevante wet- en regelgeving (harde eis);
  • Kennis van woningaanpassingen, hulpmiddelen, huishoudelijke ondersteuning en vervoer;
  • Een geldig rijbewijs B en eigen vervoer voor het uitvoeren van huisbezoeken (harde eis);
  • Ervaring met Suite4Zorg / Suite voor het Sociaal Domein is een pré;
  • Goede digitale vaardigheden;
  • Je bent in staat om wekelijks 8 tot 10 meldingen zorgvuldig af te handelen;
  • Je kunt kwaliteit en kwantiteit goed met elkaar combineren;
  • Je bent zelfstandig, snel inzetbaar en kunt je werkzaamheden goed organiseren;
  • Je beschikt over goede communicatieve vaardigheden.
