Wmo Consulent Woningaanpassingen en Voorzieningen

Als Wmo Consulent behandel je hulpvragen op het gebied van huishoudelijke hulp, rolstoelen, woon- en vervoersvoorzieningen en woningaanpassingen. Je onderzoekt wat een inwoner nodig heeft en kijkt daarbij naar de eigen kracht, zelfredzaamheid en mogelijkheden binnen het eigen netwerk.

Je gaat op huisbezoek, brengt de situatie zorgvuldig in kaart en vertaalt jouw onderzoek naar een heldere rapportage en een passend advies over maatwerkvoorzieningen. Daarbij werk je nauw samen met collega’s, bouwkundig adviseurs, zorgaanbieders, leveranciers en partners in de wijk.

Wat je gaat doen:

  • Behandelen van Wmo-aanmeldingen op het gebied van huishoudelijke hulp, rolstoelen, woonvoorzieningen, vervoersvoorzieningen en woningaanpassingen;
  • Voeren van gesprekken met inwoners om hun hulpvraag helder te krijgen;
  • Uitvoeren van huisbezoeken en onderzoeken van de persoonlijke situatie en ondersteuningsbehoefte;
  • Informeren en adviseren van inwoners over wonen, welzijn en zorg;
  • Stimuleren van eigen kracht en zelfredzaamheid;
  • Beoordelen welke ondersteuning of maatwerkvoorziening passend en noodzakelijk is;
  • Adviseren over woningaanpassingen en andere Wmo-voorzieningen;
  • Opstellen van zorgvuldige rapportages en adviezen;
  • Behandelen van complexe Wmo-casuïstiek;
  • Samenwerken en afstemmen met bouwkundig adviseurs, zorgaanbieders en leveranciers;
  • Waar nodig de verbinding leggen met partners in de wijk, zoals Buurtteams en Ouderenadviseurs;
  • Werken volgens de actuele wet- en regelgeving binnen de Wmo.

Wat jij meebrengt:

  • Een afgeronde hbo-opleiding Ergotherapie, Fysiotherapie of Verpleegkunde (harde eis);
  • Minimaal 5 jaar aantoonbare werkervaring sinds 2018 als Wmo Consulent bij een gemeente (harde eis);
  • Minimaal 5 jaar aantoonbare ervaring sinds 2018 met het uitvoeren van huisbezoeken (harde eis);
  • Minimaal 3 jaar aantoonbare ervaring sinds 2022 met woningaanpassingen (harde eis);
  • Aantoonbare actuele kennis van wet- en regelgeving binnen de Wmo (harde eis);
  • Je woont op maximaal 45 minuten rijden van Nijmegen (harde eis);
  • Meerjarige ervaring met complexe Wmo-casuïstiek is een pré;
  • Bekendheid met de omgeving Nijmegen vanuit je werk is een pré;
  • Je bent analytisch en weet tijdens een onderzoek de juiste informatie boven tafel te krijgen om tot een gedegen afweging te komen;
  • Je kunt goed aansluiten bij de behoeften van inwoners en omgaan met verschillende of conflicterende belangen;
  • Je werkt zelfstandig, neemt verantwoordelijkheid en zoekt actief de samenwerking op;
  • Je bent flexibel, collegiaal en betrouwbaar;
  • Je beschikt over goede communicatieve vaardigheden.
