Kom dan werken als jeugdzorgwerker in ons Fasehuis. Als mentor van de jongeren breng je in kaart welke doelen zij willen bereiken en zet je een passend traject in. Solliciteer nu! Wij maken graag kennis met jou.

Jouw rol, jouw uitdaging

In het Fasehuis in Arnhem wonen acht jongeren die werken aan hun zelfstandigheid. Zij kunnen 24 uur per dag terugvallen op begeleiding en gaan overdag naar school of een andere passende DA invulling. De begeleiding is individueel en afgestemd op de behoeften van de jongere. Bij de start worden tijdens een doelengesprek ontwikkeldoelen vastgesteld op het gebied van persoonlijke ontwikkeling, zelfzorg, sociale contacten en zelfstandig deelnemen aan de samenleving. Daarnaast begeleid je jongeren met psychische hulpvragen, waarbij de gedragswetenschapper de behandelinhoudelijke ondersteuning biedt.

Als jeugdzorgwerker bouw je een vertrouwensband op met de jongeren en ben je mentor van een aantal jongeren. Je begeleidt hen in hun dagelijkse leefsituatie, voert mentorgesprekken en ondersteunt hun ontwikkeling. Hierbij werk je vanuit een systeemtheoretische en oplossingsgerichte visie, stel je samen doelen op en werk je nauw samen met het netwerk van de jongere (JIM), ketenpartners en een multidisciplinair team. Daarnaast werk je actief verbindend en blijf je jezelf ontwikkelen door middel van scholing.

Samengevat bestaan je werkzaamheden uit:

  • Individueel begeleiden van jongeren en samenwerken met hun netwerk.
  • Werken vanuit een oplossingsgerichte en systeemtheoretische visie binnen een multidisciplinair team.
  • Actief verbindend werken en afstemmen met ketenpartners voor passende ondersteuning.

Jouw team, jouw plek

Het team bestaat uit ongeveer 5 collega's en het team begeleidt elk jaar een 3e jaars hbo-stagiaire. Het is een hecht en jong team, waarin nauw met elkaar wordt samengewerkt. Tijdens de doordeweekse diensten (late – en slaapdiensten) werken altijd 2 collega's met elkaar samen. Doordat het team zelf organiserend is, wordt er altijd gezocht naar een goede werkverdeling en worden gaten in het rooster zoveel mogelijk samen opgelost. Hierdoor zijn er veel mogelijkheden, waarbij flexibiliteit wel een must is. De leidinggevende is nauw betrokken bij het team, maar geeft ook veel ruimte voor eigen inbreng. Daarnaast zijn er korte lijnen met de betrokken gedragswetenschapper.

Jij bent iemand die stevig in zijn schoenen staat en met vastberadenheid onze jongeren kan begeleiden. Met jouw enthousiasme weet je hen te motiveren en perspectief te bieden. Je bent creatief in je aanpak en denkt out of the box, waardoor je jongeren kunt inspireren om nieuwe mogelijkheden te zien. Humor is voor jou een krachtig middel in het contact, en je weet het op de juiste momenten in te zetten. Daarnaast ben je een echte doener; je steekt graag de handen uit de mouwen en bent niet bang om initiatief te nemen.

Daarnaast breng je de volgende kennis en ervaring mee:

  • Een afgeronde hbo-opleiding in een relevante richting (bijv. Social Work/Pedagogiek) met SKJ-registratie. Of een afgeronde niveau 4 opleiding (bijv. sociaal werk).
  • Ervaring (of affiniteit) met psychiatrische problematiek en individueel gerichte begeleiding is een pré.
  • Kennis van of ervaring met verslavingszorg.
  • Bereidheid om onregelmatige diensten te werken.

Wat wij bieden

  • Een jaarcontract voor 28-32 uur.
  • Salaris volgens de CAO Jeugdzorg, op basis van relevante werkervaring: -Jeugdzorgwerker C (hbo met SKJ-registratie) in schaal 8: €3.055 - €4.537 (excl. vakantiegeld, ORT en eindejaarsuitkering).
  • Scholing- en trainingsaanbod om je SKJ-registratie te behouden en je verder te ontwikkelen.
