In deze rol als HBO verpleegkundige, Zorgregisseur ben jij degene die het overzicht bewaakt over het hele zorgproces. Vanaf het eerste moment dat een bewoner binnenkomt tot aan de laatste fase van zorg, ben jij betrokken en verantwoordelijk voor de samenhang en kwaliteit van de zorg.

Je staat niet alleen naast je collega’s, maar ook echt met hen in het werk. Je coacht hen in belevingsgericht werken en helpt hen om steeds beter aan te sluiten bij wat bewoners nodig hebben in hun dagelijks leven. Tegelijk ben jij een vraagbaak voor verpleegtechnische handelingen en begeleid je leerlingen in hun ontwikkeling, zodat zij met vertrouwen en vakmanschap groeien in het vak.

Naast deze coachende rol houd je scherp zicht op de kwaliteit en veiligheid van zorg. Je signaleert, stuurt bij en denkt actief mee over thema’s zoals medicatieveiligheid, valpreventie en hygiëne.

Maar je kijkt verder dan de dagelijkse praktijk. Je draagt bij aan verbetering, samenwerking en vernieuwing binnen het team. Je ziet kansen om zorgprocessen slimmer, menselijker en toekomstgerichter te maken en neemt daarin initiatief samen met je collega’s.

Wat ga je doen?

  • Coördineren van het dagelijkse zorgproces en de dagstart
  • Ondersteunen en begeleiden van leerlingen (HBO-V en MBO)
  • Bewaken van kwaliteit van zorg (o.a. medicatie, valpreventie, hygiëne)
  • Coördineren van opnames, zorgplannen en overdrachten
  • Afstemmen met artsen, behandelaren en multidisciplinaire teams
  • Verbinden van formele en informele zorg (familie, mantelzorg, vrijwilligers)
  • Signaleren en initiëren van verbeteringen in zorgprocessen
  • Meedenken over innovatie en inzet van zorgtechnologie
  • Organiseren van klinische lessen en leerontwikkeling in het team

Zo zorgen wij voor jou?

  • Salaris conform cao VVT FWG 50: € 3.772,15 – € 4.822,50 (o.b.v. 36 uur)
  • Contract voor 1 jaar met uitzicht op vast dienstverband
  • 8% vakantiegeld en 8,33% eindejaarsuitkering

Dit ben jij?

  • Diploma Verpleegkunde niveau 5/6
  • Hbo werk- en denkniveau
  • BIG-registratie
  • Brede kennis van verpleegkundige zorg (o.a. somatiek, psychiatrie, farmacologie, zorgleefplannen)
  • Ervaring met coördinatie en multidisciplinaire samenwerking
  • Sterke communicatieve en coachende vaardigheden
  • Vermogen om collega’s te motiveren en te begeleiden
  • Kennis van kwaliteitskaders, protocollen en wet- en regelgeving
  • Affiniteit met innovatie, zelfredzaamheid en belevingsgericht werken
