Heritage Home Healthcare is seeking compassionate, dependable Registered Nurses (RNs) to join our community-based nursing team in Albuquerque, New Mexico.
We currently have two RN openings serving the following areas:
- Southwest/West Albuquerque
- Outer Albuquerque and surrounding communities

What You’ll Do
As a community-based EPSDT Registered Nurse, you will conduct face-to-face supervisory visits with clients in their homes to help ensure they receive appropriate, high-quality care.
Responsibilities include:
- Conduct required supervisory home visits, generally every 60 days.
- Manage an assigned caseload of approximately 25 clients.
- Assess clients’ overall well-being and identify concerns or changes in condition.
- Communicate with clients, families, caregivers, and the care team.
- Provide education, guidance, and appropriate resources.
- Complete accurate and timely visit documentation.
- Communicate concerns and recommendations to appropriate team members.
- Maintain compliance with applicable nursing standards, company policies, and documentation requirements.
- Work independently while maintaining communication with the clinical team.
Service Areas & Travel
We are currently hiring Registered Nurses to support two Albuquerque-area territories.
Territory 1: Southwest/West Albuquerque
Provide community-based nursing visits to clients throughout southwest and west Albuquerque.
Territory 2: Outer Albuquerque and Surrounding Communities
Provide community-based nursing visits to clients in the outer Albuquerque area and surrounding communities.
Candidates will have the opportunity to discuss their preferred coverage area during the screening process.
Because this is a community-based position, nurses must be comfortable traveling throughout their assigned territory.
Mileage is reimbursed at the current IRS standard mileage rate.
Occasional travel outside the primary assigned area may be required based on client needs.
What We’re Looking For
Required Qualifications:
- Current, valid New Mexico Registered Nurse (RN) license.
- Minimum of one year of nursing experience.
- Valid driver’s license and reliable transportation.
- Strong assessment and documentation skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage a flexible schedule.
- Comfortable conducting visits in clients’ homes.
- Ability to travel throughout the assigned service area.
Preferred Experience:
Experience in any of the following areas is helpful but not required:
- Home health
- Community health
- Case management
- Pediatrics
- Patient education
- Care coordination
Nurses from a variety of clinical backgrounds are encouraged to apply.
