In de functie van wijkverpleegkundige mag jij jouw expertise inzetten om ouderen zo lang mogelijk zelfstandig thuis te laten wonen. Als wijkverpleegkundige op Goeree-Overflakkee krijg je de ruimte om zelfstandig beslissingen te nemen, de juiste zorg te organiseren en écht het verschil te maken. Je kijkt verder dan de zorgvraag van vandaag, houdt regie en werkt samen met collega’s aan passende zorg. Met jouw kennis, daadkracht en persoonlijke aandacht geef je mensen de regie over hun eigen leven terug.

Dit ga je doen als Wijkverpleegkundige

Als wijkverpleegkundige bied je zorg op maat in de thuissituatie, waarbij je zowel de medische zorg als het welzijn van de cliënt versterkt. Je kijkt naar de zorgbehoefte, stelt vast welke ondersteuning nodig is en houdt overzicht over de uitvoering van de zorg.

Concreet betekent dat:

  • Het vaststellen van de behoefte aan verpleegkundige zorg door middel van klinisch redeneren.
  • Breng je de zorgbehoefte van cliënten in kaart, stel je verpleegkundige indicaties en coördineer je de benodigde zorg.
  • Signaleer je veranderingen in de gezondheid en stimuleer je cliënten om zo lang mogelijk zelfstandig te blijven, met aandacht voor preventie en welzijn.
  • In samenwerking met huisartsen, familieleden, collega's en andere zorgorganisaties stem je de zorg goed op elkaar af.
  • Daarnaast adviseer en ondersteun je collega's bij verpleegkundige vraagstukken en draag je bij aan de kwaliteit van zorg in de wijk.

Wat je van ons mag verwachten

  • Salaris volgens cao VVT, FWG 50, €3.592,96 tot €4.822,50 (o.b.v. 36 uur). Mooi vooruitzicht: jouw salaris stijgt per 1 juni 2027 met 3,6% en per 1 juni 2028 opnieuw met 3,6%!
  • Direct een vast contract en je kiest zelf de omvang van je contract van 24 tot 36 uur per week, afgestemd op jouw wensen en in overleg.
  • Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.
  • Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.
  • Toegang tot o.a. HeyCoach, budgetcoach en DigiCoach voor jouw ontwikkeling.

Wat jij meebrengt

  • Een hbo-diploma Verpleegkunde en geldige BIG-registratie.

Over het team

Je komt terecht in het groeiende Team Goeree West, een betrokken team van 16 collega’s, onder wie twee wijkverpleegkundigen. Je valt onder de Manager Zorg en werkt nauw samen met collega’s, specialisten en huisartsen. Samen zetten jullie je in voor goede zorg, van Ouddorp tot Middelharnis. Je werkt nauw samen met collega’s, specialisten en huisartsen.

Om de veiligheid van onze cliënten en medewerkers te waarborgen, is voor alle medewerkers die bij Careyn in dienst komen een Verklaring Omtrent Gedrag (VOG) vereist. Bij gelijke geschiktheid krijgen interne sollicitanten voorrang ten opzichte van externe kandidaten.
