Zorgeloos thuis staat voor meer dan thuiszorg alleen. Het zorgeloos thuis team staat voor optimale zorg voor cliënten die graag thuis blijven wonen en dat betekent dat er ook tijd is voor een kopje thee of een wandeling. Wat krijg je van ons?

  • Een jaarcontract (24 - 36 uur) met uitzicht op een vast dienstverband;
  • Salaris is conform CAO VVT in FWG 45 min. € 3.157,- en max. € 4.246,- bruto bij een dienstverband van 36 uur;
  • Eindejaarsuitkering van 8,33% van het bruto jaarinkomen en vakantietoeslag van 8% per jaar;
  • Pensioenopbouw via Pensioenfonds Zorg & Welzijn, collectiviteitskorting op je zorgverzekering en reiskostenvergoeding conform CAO VVT;
  • Cordaan stimuleert persoonlijke ontwikkeling door het aanbieden van gerichte trainingen, cursussen en scholing via o.a. Good Habitz;
  • Een bonus van €500,- netto bij het aandragen van een collega;
  • Een fietsplan en fietsreparatievergoeding;

Wat ga je doen? Als cliëntcoach (verpleegkundige of verzorgende IG) ga je aan de slag binnen een van onze Zorgeloos Thuis teams in een wijk in Amsterdam. Samen met je collega’s verzorgende C, verzorgende IG en een collega-cliëntcoach regelen jullie alles van A tot Z voor de cliënt. Je verricht verzorgende en verpleegtechnische handelingen bij de cliënt en coördineert daarnaast de directe zorg en de dienstverlening hieromheen. Of het nu gaat om het bestellen van materialen of samen een boodschapje met de cliënt, jij bent de aangewezen persoon.

Je voert samen met de wlz - verpleegkundige de intakegesprekken bij nieuwe cliënten. Jij kent als cliëntcoach het levensverhaal van de cliënt en brengt het netwerk van de cliënt in kaart. Je onderhoudt de contacten met de familie, partner en andere mantelzorgers. Daarnaast signaleer je of de gestelde indicatie nog voldoet aan de zorg die de cliënt nodig heeft.

Tenslotte heb je een coachende rol naar het team, je ondersteunt hen zodat zij de best mogelijke zorg kunnen leveren. Samen met je collega-cliëntcoach ben je betrokken bij het plannen & roosteren en organiseer je de overleggen met het team om de cliënten te bespreken en werkafspraken te maken.

Wie ben jij?

  • Je hebt een diploma verzorgende IG of verpleegkunde mbo-4;
  • Je bent flexibel in te zetten in de wijk (maandag t/m zondag);
  • Je hebt ervaring met coördinerende taken;
  • Je werkt graag zelfstandig en staat stevig in je schoenen;
  • Je bent sociaal en kunt je goed inleven in een ander.
