Begeleider - 30-09-2026 - Den Bosch - fulltime - hbo - vast Wat ga je doen? Als Begeleider ouderenzorg bij Noorderkroon begeleid je bewoners met complexe psychische en/of psychosociale problematiek in hun dagelijks leven. Je observeert gedrag, verricht anamnese en onderzoek en vertaalt je bevindingen samen met de cliënt en diens netwerk naar een passend deelplan. Dit bespreek je binnen het multidisciplinaire behandelteam. Je begeleidt cliënten methodisch en doelgericht op het gebied van onder andere zelfregie, dagelijks functioneren en resocialisatie. In acute of crisissituaties blijf je rustig en handel je adequaat. Je schakelt waar nodig een verpleegkundige of gedragsdeskundige in en verleent indien nodig ook somatische zorg. Daarnaast coördineer en bewaak je de begeleiding. Je vertegenwoordigt de cliënt in overleggen met andere zorgverleners, zorgt voor een goede overdracht en rapporteert zorgvuldig. Ook denk je mee over het verbeteren van werkwijzen en zorgprogramma s en organiseer je activiteiten die bijdragen aan het welzijn van cliënten. Wie ben jij? Je beschikt over een diploma mbo 4 Maatschappelijke Zorg en hebt ruime ervaring met cliënten met complexe psychiatrische en/of psychosociale problematiek. Je bent bekend met relevante methodieken, protocollen en de sociale kaart en hebt ervaring met het coördineren van zorg. Daarnaast: - Pas je methodisch werken toe en werk je volgens geldende protocollen en de sociale kaart - Coördineer en bewaak je de zorg rondom cliënten - Werk je zelfstandig, stel je prioriteiten en neem je verantwoordelijkheid - Blijf je rustig en daadkrachtig in acute- en crisissituaties - Communiceer je helder en weet je cliënten te motiveren en te begrenzen - Observeer en signaleer je tijdig risico s en veranderingen - Werk je systematisch, zorgvuldig en leg je afspraken en voortgang goed vast Wat bieden wij jou?
