Begeleider forensisch tijdelijk - - Huizen - € 2.888 - € 3.917 per maand - Team Rijsbergenweg Huizen Bij onze locatie begeleid je volwassenen met diverse problematieken, waaronder psychische en forensische zorg. Samen met een betrokken team van 15 collega's zorg je voor een veilige leefomgeving. Functieomschrijving Als begeleider ondersteun je cliënten in hun dagelijks leven en help je hen stap voor stap in hun herstelproces. Je kijkt naar wat iemand wél kan en biedt begeleiding die aansluit bij de mogelijkheden, behoeften en doelen van de cliënt. Je werkt volgens het begeleidingsplan en levert een actieve bijdrage aan de evaluaties hiervan. Je signaleert als begeleider veranderingen in gedrag en welzijn en hebt oog voor veiligheid, inclusief het herkennen van eventueel delictgedrag of delictpatronen. Deze signalen bespreek je met collega's en leg je vast in rapportages. Samen met het team zorg je voor een veilige, prettige en stabiele leefomgeving voor de cliënten. Bij crisissituaties weet je als begeleider adequaat te handelen. Voor deze functie is beschikbaarheid op vrijdagavond een vereiste. Daarnaast draai je bij voorkeur ook avonddiensten, waarbij hierover afstemming en overleg mogelijk is. Wij zijn opzoek naar iemand voor 28 tot 36 uur per week. Het gaat om een tijdelijke vacature in verband met zwangerschapsverlof. De startdatum is in overleg rond augustus. Jouw werkplek De locatie Rijsbergenweg heeft op de locatie 20 cliënten wonen, verdeeld over 4 woningen. In totaal wordt er aan 35 cliënten begeleiding geboden vanuit deze locatie. Wij bieden intramurale en ambulante hulpverlening aan een WLZ, WMO en forensische doelgroep. Steeds meer cliënten op deze locatie hebben een forensische titel. Ze komen vanuit detentie, hebben een TBS maatregel en/of zijn bekend bij de reclassering. De doelgroep bestaat uit volwassen cliënten met diverse problematieken. Dit kan zijn psychisch- en/of psychische sociale problematiek, verslavings...
