Hospiceverpleegkundige - Jouw werkzaamheden - Wat wij zoeken - Wat wij bieden Wat jij gaat doen In het Sint Jans hospice De Casembroot kunnen vijf bewoners hun laatste levensdagen doorbrengen in een warme, huiselijke sfeer. Als hospiceverpleegkundige ben jij er voor hen én hun naasten. Je biedt deskundige zorg, maar minstens zo belangrijk: je bent er op de momenten die ertoe doen. Samen met je collega-verpleegkundigen en vrijwilligers zorg je 24 uur per dag voor onze bewoners. Het verpleegkundig team werkt zelfsturend. Vanuit jouw kwaliteiten, ervaring en interesses heb je binnen het team een eigen rol en verantwoordelijkheid. De zorg in ons hospice is gericht op het ondersteunen van bewoners en hun naasten in een kwetsbare en ingrijpende periode. Je kijkt verder dan alleen de lichamelijke zorg en hebt aandacht voor wat iemand op dat moment nodig heeft. Daarbij werk je nauw samen met verpleegkundigen, vrijwilligers en andere disciplines. Onze bewoners worden medisch begeleid door een team van hospiceartsen en/of hun eigen huisarts. Als hospiceverpleegkundige combineer je verpleegkundige en coördinerende taken. Je biedt hoogwaardige palliatieve zorg en begeleidt bewoners en hun naasten op psychosociaal gebied. Je hebt oog voor de lichamelijke, emotionele, sociale en spirituele behoeften die een rol kunnen spelen in de laatste levensfase. Je stelt prioriteiten in de dagelijkse zorgverlening, plant en coördineert verpleegkundige interventies en evalueert voortdurend de effecten daarvan. Je zorgt voor een zorgvuldige rapportage en overdracht en neemt deel aan multidisciplinaire overleggen. Samen met onder andere de hospicearts, de eigen huisarts van de bewoner en de geestelijk verzorger stem je de zorg zo goed mogelijk af. Ook de vrijwilligers zijn een belangrijk onderdeel van het hospice. Tijdens je diensten werk je intensief met hen samen. Je ondersteunt en begeleidt hen waar nodig en draagt bij aan een prettige en profession...
