Opdrachtomschrijving De Gemeente Ede zoekt een Specialistisch Ondersteuner Jeugd (SOJ) ter versterking van de integrale aanpak van jeugd-ggz problematiek binnen de eerstelijnszorg. De SOJ wordt gekoppeld aan meerdere huisartsenpraktijken binnen een vastgesteld werkgebied en biedt laagdrempelige, vroegtijdige en gecoördineerde ondersteuning aan jeugdigen van 0 tot 18 jaar met psychische en/of psychosociale problematiek. Daarnaast werkt de SOJ mee aan een pilot waarin we de SOJ-functie verbreden naar opvoedkundige vragen in samenwerking met een HBO-professional. De SOJ biedt ondersteuning aan huisartsenpraktijken en is verantwoordelijk voor het: In kaart brengen van de hulpvraag en problematiek van jeugdigen (in max. 5 gesprekken); Opstellen van een passend advies of ondersteuningsplan; Bieden van kortdurende behandeling en begeleiding; Geven van gezondheidsvoorlichting en psycho-educatie; Uitvoeren van triage en screening met het oog op verwijzing; Stimuleren van zelfredzaamheid bij jeugdigen en hun systeem; Ondersteunen van de huisarts bij diagnostiek, behandelvoorstellen en doorverwijzing; Nauw samenwerken met gemeentelijke teams (Centrum voor Jeugd en Gezin, ToegangsTeam Jeugd, Sociaal Team) en andere betrokken professionals (scholen, jeugdarts, verslavingszorg e.a.); De SOJ werkt met een hoge mate van zelfstandigheid in nauwe samenwerking met de huisarts. De inzet van de SOJ resulteert in: Snel toegankelijke, passende en kwalitatieve zorg voor jeugdigen van 0 tot 18 jaar met opvoedproblematiek, ...
