Le poste

Le service social détache des assistants sociaux dans chaque unité de soins.

Les unités sont au nombre de 10 et ont chacune un projet thérapeutique spécifique en lien avec les patients qui y sont accueillis.

  • L'assistant.e social.e au CHJT travaille à la mise en place et au maintien des acquis sociaux de la personne. De l'admission jusqu'à la sortie, il.elle joue un rôle essentiel dans l'accueil et l'accompagnement pluridisciplinaire de la personne soignée.
  • L'assistant.e social.e est un.e acteur.rice majeur.e de la création et/ou du maintien du réseau de la personne hospitalisée. En cela, il.elle est le lien privilégié entre la famille, le réseau et les équipes de soins.
  • L'assistant.e social.e fera partie du service social du CHJT, un service dynamique offrant ses propres espaces de réflexions et de mise en place de projets. L'équipe sociale s'inscrit activement dans le développement de la nouvelle approche thérapeutique soutenue par le CHJT (https://apertio.be/fr) en modernisant ses pratiques et en effectuant notamment un grand travail de développement des réseaux.

Formation requise :

  • Titulaire d'un diplôme d'assistant A1 accompagné d'une expérience en santé mentale.

Compétences :

  • Compétences rédactionnelles, de synthèse et de transmission d'informations
  • Travail en équipe pluridisciplinaire et en réseau
  • Bonne connaissance des procédures et des législations sociales en vigueur
  • Connaissance des outils informatiques nécessaires à la fonction
  • Atouts supplémentaires : La connaissance des contextes médico-légaux, le bilinguisme français/néerlandais, le permis B
