Ter versterking van ons domein Sociaal Domein zijn we op zoek naar een ervaren Verandermanager Wmo en Jeugd. We zoeken een gedreven professional die voor onze detacheringsprojecten: - Leidinggeeft aan medewerkers, coördinatoren en gedragswetenschappers van Team Toegang - Zorgt voor rust, focus, duidelijkheid en een veilige omgeving waarin medewerkers kunnen leren en zich ontwikkelen - Vertaalt de gemeentelijke visie en de principes van Stevige Lokale Teams naar het dagelijks handelen en nieuwe werkwijzen - Verduidelijkt rollen, mandaten, escalatieroutes en besluitvorming binnen het team - Signaleert wat niet werkt, neemt de regie en zorgt dat verbeteringen worden doorgevoerd - Versterkt de samenwerking met de manager bedrijfsvoering en draagt kennis over, zodat het team op termijn zelfstandig kan functioneren - Ontwikkelt het team tot een robuuste en toekomstbestendige toegang tot jeugdhulp en Wmo-begeleiding - Zorgt dat formatie, functies, rolverdeling, kennis en expertise aansluiten op de benodigde ondersteuning en de zwaarte van de casuïstiek - Werkt samen met beleid, het lokale specialistische team, voorveldpartijen en gecontracteerde aanbieders aan samenhangende ondersteuning voor inwoners Je draagt bij aan een stevig lokaal team dat inwoners passende ondersteuning biedt en samenwerkt aan preventie, vroegsignalering en een sterke sociale basis.
