Consulent Beschermd Wonen

Als Consulent Beschermd Wonen ondersteun je inwoners die vanwege complexe problematiek intensieve ondersteuning nodig hebben bij het wonen en op verschillende levensgebieden. Het gaat onder andere om inwoners met psychische kwetsbaarheden of een verstandelijke beperking die 16 tot 24 uur per dag toezicht in de nabijheid nodig hebben.

Je onderzoekt wat er daadwerkelijk speelt achter de hulpvraag en brengt de situatie integraal in kaart. Op basis van gesprekken, huisbezoeken en zorgvuldig onderzoek bepaal je welke ondersteuning passend is. Je voert niet zelf de dagelijkse begeleiding uit, maar houdt de regie over het proces en bewaakt de voortgang, kwaliteit en gemaakte afspraken.

Wat je gaat doen:

  • Je onderzoekt meldingen en aanvragen voor beschermd wonen;
  • Je voert gesprekken en huisbezoeken uit om de hulpvraag en persoonlijke situatie zorgvuldig in kaart te brengen;
  • Je onderzoekt de zelfredzaamheid, psychische problematiek, veiligheid en het sociale netwerk van inwoners;
  • Je voert veiligheids- en risico-inschattingen uit;
  • Je beoordeelt welke ondersteuning en maatwerkvoorziening passend is;
  • Je stelt rapportages op en zorgt voor zorgvuldige en actuele dossiervorming;
  • Je stelt beschikkingen op voor het toekennen of afwijzen van ondersteuning;
  • Je bepaalt de duur en zwaarte van indicaties;
  • Je voert de regie over ondersteuningstrajecten en bewaakt de voortgang en afgesproken doelen;
  • Je onderhoudt contact met inwoners, zorgaanbieders en andere betrokken partijen;
  • Je coördineert de samenwerking met onder andere GGZ, Veilig Thuis, verslavingszorg, reclassering en woningcorporaties;
  • Je monitort de kwaliteit en resultaten van de ingezette ondersteuning;
  • Je signaleert knelpunten en stuurt waar nodig bij;
  • Je zorgt voor een goede afstemming tussen alle betrokken partijen rondom de inwoner.

Wat jij meebrengt:

  • Je hebt minimaal 3 jaar aantoonbare werkervaring als Wmo-consulent bij een gemeente (harde eis);
  • Je hebt minimaal 1 jaar aantoonbare werkervaring als Consulent Beschermd Wonen bij een gemeente (harde eis);
  • Je beschikt over een afgeronde hbo-bacheloropleiding (harde eis);
  • Minimaal 3 jaar werkervaring als Consulent Beschermd Wonen bij een gemeente is een pré;
  • Een afgeronde hbo-bacheloropleiding in de richting Social Work of MWD is een pré;
  • Ervaring met Suite4SociaalDomein is een pré;
  • Je bent bij voorkeur uiterlijk 1 december 2026 beschikbaar voor het gevraagde aantal uren;
  • Je hebt ervaring met complexe hulpvragen en kunt verschillende levensgebieden integraal beoordelen;
  • Je bent in staat om zelfstandig onderzoek uit te voeren en tot een zorgvuldig onderbouwd besluit te komen;
  • Je kunt regie voeren op complexe ondersteuningstrajecten en effectief samenwerken met verschillende ketenpartners;
  • Je bent analytisch, zorgvuldig en communicatief sterk;
  • Je bent bereid om minimaal 50% van je werktijd op locatie te werken. Hybride werken is in goed overleg mogelijk.
