Consulent Wmo Begeleiding

Als Consulent Wmo ben je het eerste aanspreekpunt voor inwoners die ondersteuning nodig hebben vanuit de Wet maatschappelijke ondersteuning. Je onderzoekt wat er daadwerkelijk speelt, kijkt verder dan de eerste hulpvraag en beoordeelt welke ondersteuning of voorziening passend is.

Je behandelt zowel enkelvoudige als complexe aanvragen en hebt daarbij specifiek kennis van begeleidingsvraagstukken. Je werkt nauw samen met collega’s binnen Zorg en Welzijn en draagt vanuit de uitvoering bij aan het signaleren van knelpunten en verbeterkansen binnen het Wmo-beleid.

Wat je gaat doen:

  • Je informeert en adviseert inwoners over de Wmo en mogelijke voorliggende voorzieningen;
  • Je neemt Wmo-aanvragen in behandeling en beoordeelt of deze worden toegekend of afgewezen;
  • Je voert brede vraagverheldering uit om de daadwerkelijke ondersteuningsbehoefte van inwoners vast te stellen;
  • Je behandelt zowel enkelvoudige als complexe aanvragen;
  • Je legt zelfstandig huisbezoeken af en voert gesprekken met inwoners en andere betrokkenen;
  • Je houdt bij complexe situaties rekening met andere leefgebieden en schakelt waar nodig met andere domeinen;
  • Je behandelt begeleidingsvraagstukken en werkt daarbij met het normenkader van Factum;
  • Je draagt bij aan een optimale inzet van het normenkader en de beschikbare software;
  • Je signaleert samen met beleidsmedewerkers knelpunten en verbetermogelijkheden binnen beleid en verordeningen;
  • Je legt gesprekken en onderzoeksresultaten zorgvuldig vast in de systemen;
  • Je vertaalt je onderzoek en beoordeling naar kwalitatief goede beschikkingen.

Wat jij meebrengt:

  • Je hebt minimaal 2 jaar aantoonbare werkervaring als Wmo-consulent binnen een gemeente (harde eis);
  • Je hebt ervaring met het behandelen en beoordelen van Wmo-aanvragen;
  • Je hebt kennis van en ervaring met begeleidingsvraagstukken;
  • Je bent in staat zelfstandig huisbezoeken en brede vraagverhelderende gesprekken uit te voeren;
  • Je kunt zowel enkelvoudige als complexe ondersteuningsvragen behandelen;
  • Je kunt zorgvuldig rapporteren en kwalitatief goede beschikkingen opstellen;
  • Je woont op maximaal 55 minuten reistijd zonder file van Opmeer (harde eis).
