Consulent Wmo

Als Consulent Wmo werk je met verschillende doelgroepen en uiteenlopende zorgvraagstukken. Je voert de regie over zowel reguliere als complexe Wmo-casuïstiek, waarbij de nadruk onder andere ligt op begeleiding, beschermd wonen en woningaanpassingen.

Je hebt veel contact met inwoners, zorgaanbieders en collega’s binnen verschillende disciplines. Daarbij weet je snel te schakelen, houd je het hoofd koel en durf je zelfstandig beslissingen te nemen. Je werkt binnen een zelforganiserend team waarin collega’s elkaar ondersteunen en gezamenlijk verantwoordelijk zijn voor goede dienstverlening aan inwoners.

Wat je gaat doen:

  • Je voert gesprekken met inwoners, zorgaanbieders en collega’s over Wmo-vraagstukken;
  • Je behandelt complexe hulpvragen op het gebied van begeleiding;
  • Je behandelt vraagstukken en indicaties rondom beschermd wonen;
  • Je behandelt aanvragen en vraagstukken rondom woningaanpassingen;
  • Je voert de regie over jouw dossiers en zorgt voor passende oplossingen voor inwoners;
  • Je beoordeelt welke voorzieningen en ondersteuning aansluiten bij de hulpvraag van de inwoner;
  • Je verzorgt de administratieve verwerking en vastlegging van jouw werkzaamheden;
  • Je bent bereikbaar voor inwoners die ondersteuning nodig hebben, zowel telefonisch als op locatie;
  • Je schakelt met collega’s van verschillende afdelingen wanneer een hulpvraag meerdere disciplines raakt;
  • Je signaleert kansen en knelpunten binnen het werkgebied en bespreekt deze met het team;
  • Je werkt zelfstandig binnen je eigen caseload en zoekt tegelijkertijd actief de samenwerking met collega’s.

Wat jij meebrengt:

  • Je beschikt over minimaal hbo werk- en denkniveau (harde eis);
  • Je hebt minimaal 5 jaar aantoonbare werkervaring als Consulent Wmo (harde eis);
  • Je hebt aantoonbare ervaring met complexe hulpvragen op het gebied van begeleiding en beschermd wonen (harde eis);
  • Je bent bekend met verschillende Wmo-voorzieningen en kunt passende ondersteuning beoordelen;
  • Je hebt ervaring met woningaanpassingen;
  • Je bent een allround Wmo-consulent en kunt verschillende soorten Wmo-casuïstiek zelfstandig behandelen;
  • Je kunt snel schakelen en houdt ook bij complexe of onverwachte situaties het hoofd koel;
  • Je durft zelfstandig beslissingen te nemen en knopen door te hakken;
  • Je bent dienstverlenend, oplossingsgericht en betrokken bij de inwoners die je ondersteunt;
  • Je werkt prettig samen binnen een zelforganiserend team en neemt verantwoordelijkheid voor jouw bijdrage aan het gezamenlijke resultaat.
