Sociaal Werker Wmo

Als Sociaal Werker Wmo ben je verantwoordelijk voor de uitvoering van de Wmo en ondersteun je inwoners bij uiteenlopende en soms complexe ondersteuningsvragen. Je voert zorgvuldig onderzoek uit, waarbij je niet alleen kijkt naar de hulpvraag, maar ook naar voorliggende voorzieningen, de eigen kracht van de inwoner en de mogelijkheden binnen het eigen netwerk.

Je doorloopt het onderzoek aan de hand van vijf stappen en onderbouwt jouw bevindingen vanuit het juridische kader. Op basis daarvan stel je een gespreksverslag op met een zwaarwegend advies. Je werkt toekomstgericht, denkt in oplossingen en weet wanneer een complexe situatie vraagt om overleg met collega’s of bespreking tijdens een casuïstiekbijeenkomst.

Wat je gaat doen:

  • Uitvoeren van Wmo-onderzoeken aan de hand van de vijf stappen;
  • Analyseren en beoordelen van ondersteuningsvragen van inwoners;
  • Onderbouwen van jouw onderzoek en advies vanuit het geldende juridische kader;
  • Opstellen van gespreksverslagen met een zorgvuldig onderbouwd en zwaarwegend advies;
  • Onderzoeken van de eigen kracht en mogelijkheden binnen het netwerk van de inwoner;
  • Meewegen van voorliggende voorzieningen voordat een Wmo-maatwerkvoorziening wordt ingezet;
  • Zelfstandig bepalen van prioriteiten binnen de binnenkomende Wmo-vraagstukken;
  • Behandelen van complexe Wmo-casuïstiek;
  • Signaleren van knelpunten en deze tijdig bespreken met collega’s;
  • Inbrengen en bespreken van complexe casuïstiek tijdens casuïstiekbijeenkomsten;
  • Creatief en doelmatig zoeken naar passende oplossingen voor inwoners;
  • Stimuleren van de eigen regie van inwoners en gezinnen zonder werkzaamheden onnodig over te nemen;
  • Actief samenwerken met collega’s en bijdragen aan een goede teamdynamiek;
  • Zorgvuldig verwerken van werkzaamheden in i-Sam.

Wat jij meebrengt:

  • Een afgeronde relevante hbo-opleiding, bijvoorbeeld Social Work of een vergelijkbare opleiding (harde eis);
  • Minimaal 5 jaar recente en relevante werkervaring binnen de Wmo (harde eis);
  • Ervaring met werken in i-Sam (harde eis);
  • Een geldig rijbewijs en de beschikking over een auto (harde eis);
  • Een geldige VOG die maximaal 3 maanden oud en relevant is voor de functie, of de bereidheid deze aan te vragen (harde eis);
  • Ruime werkervaring binnen een gemeentelijke context is een pré;
  • Je verstaat het Fries; het kunnen spreken van Fries is een pré;
  • Je beschikt over voldoende juridische kennis om Wmo-onderzoeken en adviezen zorgvuldig te onderbouwen;
  • Je kunt zelfstandig complexe ondersteuningsvragen analyseren en prioriteren;
  • Je werkt vanuit de eigen kracht en mogelijkheden van de inwoner;
  • Je bent proactief, oplossingsgericht en weet wanneer afstemming met collega’s nodig is;
  • Je kunt zowel zelfstandig als in teamverband goed functioneren;
  • Je communiceert helder en draagt actief bij aan een prettige samenwerking binnen het team.
