Wmo Consulent Beschermd Wonen

Als Wmo Consulent Beschermd Wonen werk je binnen het regionale toegangsteam Beschermd Wonen in Flevoland. Je richt je op inwoners vanaf 18 jaar met psychiatrische, verslavings-, psychosociale en/of LVB-problematiek die nog niet volledig zelfstandig kunnen wonen en functioneren. Samen met Wmo-consulenten uit vijf gemeenten ben je verantwoordelijk voor een zorgvuldige toegangsbepaling tot Beschermd Wonen.

Je onderzoekt wat er daadwerkelijk nodig is en kijkt daarbij breder dan alleen Beschermd Wonen. Mogelijk past individuele begeleiding, dagbesteding, ondersteuning vanuit de lokale Wmo of een andere voorziening beter. Je beheert zelfstandig je caseload van melding tot beschikking en komt op basis van zorgvuldig onderzoek tot een goed gemotiveerd advies en besluit.

Wat je gaat doen:

  • Beoordelen van aanmeldingen voor Beschermd Wonen op volledigheid;
  • Zelfstandig beheren van je caseload van melding tot beschikking;
  • Voeren van gesprekken met cliënten en verwijzers om de ondersteuningsvraag te verhelderen;
  • Onderzoeken welke vorm van ondersteuning het meest passend is;
  • Afwegen van mogelijkheden zoals individuele begeleiding, dagbesteding, Beschermd Wonen en ondersteuning vanuit de lokale Wmo;
  • Beoordelen van dossiers van inwoners met psychiatrische, verslavings-, psychosociale en/of LVB-problematiek;
  • Afstemmen met specialisten wanneer aanvullende expertise nodig is;
  • Verwerken en zorgvuldig registreren van onderzoeksgegevens in het systeem;
  • Adviseren van het multidisciplinair overleg over het te nemen besluit;
  • Opstellen en motiveren van positieve en negatieve adviezen rondom de toegang tot Beschermd Wonen of de lokale Wmo;
  • Samenwerken met Wmo-consulenten en andere professionals uit de vijf gemeenten;
  • Afstemmen met partners binnen en buiten de regio Sociaal Domein Flevoland;
  • Signaleren van knelpunten in werkprocessen en deze bespreken binnen het team;
  • Bijdragen aan de verdere inrichting en implementatie van het regionale toegangsproces Beschermd Wonen.

Wat jij meebrengt:

  • Een afgeronde relevante hbo-opleiding, bijvoorbeeld HBO-V, Maatschappelijk Werk of SPH (harde eis);
  • Minimaal 5 jaar aantoonbare werkervaring met de doelgroep (harde eis);
  • Actuele kennis van het sociaal domein en ontwikkelingen binnen het zorglandschap, specifiek op het gebied van de Wmo;
  • Inhoudelijke kennis van de Wmo en overige relevante wet- en regelgeving;
  • Ervaring met inwoners met psychiatrische, verslavings-, psychosociale en/of LVB-problematiek;
  • Paramedische kennis om dossiers te kunnen beoordelen en onderbouwde beslissingen te nemen;
  • Kennis van de sociale kaart van Flevoland en de lokale gemeenten is een pré;
  • Je kunt zelfstandig onderzoek uitvoeren en tot een zorgvuldig gemotiveerd advies en besluit komen;
  • Je werkt procesmatig en analytisch en weet snel overzicht te creëren;
  • Je kunt goed plannen, prioriteiten stellen en zelfstandig processen bewaken;
  • Je beschikt over daadkracht, doorzettingsvermogen en een praktische instelling;
  • Je hebt goede samenwerkingsvaardigheden en legt gemakkelijk verbinding met wijkteams en andere partners;
  • Je hebt affiniteit met administratieve processen en digitale registratiewerkzaamheden en werkt hierin zorgvuldig;
  • Je beschikt over goede mondelinge en schriftelijke communicatieve vaardigheden;
  • Je kunt goed luisteren en weet de vraag achter de vraag te achterhalen;
  • Je hebt een flexibele, open en collegiale werkhouding.
