Zet jij gezinnen in hun eigen kracht en wil je graag een cruciale rol hebben in het verbeteren van het welzijn van jeugd én hun ouders? Heb jij de power om rust te brengen in complexe situaties en koers te houden wanneer het spannend wordt? Dan is dit jouw plek binnen de wijkteams van Rotterdam.

Wat ga je doen?

Als Jeugd- en gezinscoach binnen de maatschappelijke wijkteams van [job.werklocatie] zet jij je actief in om het functioneren van aangemelde gezinnen te verbeteren. Je regisseert, organiseert en biedt zorg en ondersteuning in situaties waarbij er verhoogde spanning is in het gezin/systeem. Je legt contact met de cliënt en de opvoeders en brengt problemen en zorgbehoefte in kaart. Je signaleert bijzonderheden, voert veiligheidstaxaties uit en betrekt het netwerk van de cliënt. Samen kom je tot een plan van aanpak.

Je motiveert en stimuleert de cliënt, monitort het hulpverleningsproces en biedt zowel collectieve als individuele ondersteuning. Bij crisissituaties zoals acute psychische nood of een instabiele situatie grijp je in: je zorgt dat de juiste hulpverlening wordt ingezet. Je onderhoudt een relevant netwerk t.b.v. de afstemming en de inzet van (informele) zorg. Je legt contacten met (sociale)netwerken in de wijk.

Wie ben jij?

  • Je hebt een agogisch hbo-diploma (bijvoorbeeld MWD, SPH, Social Work) én bent SKJ-geregistreerd (vermeld je registratienummer op je cv).
  • Je hebt meerdere jaren relevante werkervaring in de jeugdzorg.
  • Je hebt kennis van de doelgroepen en de vaak gecombineerde problematiek die daarbij komt kijken.
  • Je hebt oog voor diversiteit en houdt hier rekening mee.
  • Je gaat goed om met tegengestelde belangen en blijft dan ook in verbinding.
