  • Als Cliëntbegeleider een bruto maandsalaris van maximaal € 3.635,00 (FWG 35) op basis van een 36-urige werkweek en arbeidsvoorwaarden conform cao Gehandicaptenzorg;

Hier kom je te werken: Dolomieten 9 bevindt zich midden in een woonwijk in Assendelft. Er wonen op deze locatie 8 cliënten in de leeftijdscategorie van 52 t/m 75 jaar. Cliënten wonen op Dolomieten 9 voor hun individuele zorgvraag waarbij er gebruik gemaakt wordt van 2 zorgbadkamers. De nadruk ligt vooral in het behouden van wat de cliënt nog kan en zo nodig tot overname van de adl-handelingen.

Er wordt gewerkt met tilliften en douchestoel en douche brancard. De ouder wordende cliënt en de daarbij komende problematiek (dementie, lichamelijke achteruitgang) staat centraal naast de ziektebeelden epilepsie, autisme en slikproblematiek. Er wordt ook gewerkt met palliatieve markering van een aantal cliënten. Binnen de woning wordt er individueel dagbeleving aangeboden naar gelang behoefte van de cliënt. Dolomieten beschikt over een droge en een natte snoezelruimte en vinden er wekelijks activiteiten plaats zoals muziek, en beweging.

Tijdens de nacht is er een wakkere nachtdienst in de woonvoorziening aanwezig.

Dit ben jij:

Je bent een ondernemende, zelfstandige en fijne collega met oog voor de cliënt. Wij houden ons bezig met de dagelijkse organisatie, zorg, ondersteuning en begeleiding van onze gewaardeerde cliënten. Je vult de individuele plannen van onze cliënten met toewijding en creativiteit in. Onze cliënten verlangen vaak naar zekerheid en duidelijkheid, jij bent de sleutel tot het bieden van die geruststelling. Als Cliëntbegeleider draag je bij aan een harmonieuze omgeving, wat onze onderlinge samenwerking versterkt. Binnen ons team hechten we veel waarde aan kernwaarden zoals openheid, collegialiteit, de mogelijkheid om te groeien en betrouwbaarheid.

Als Cliëntbegeleiders draag je zorg dat de cliëntplannenplannen goed worden uitgevoerd en opgevolgd.

Daarnaast heb jij:

  • Als Cliëntbegeleider een afgeronde zorgopleiding op minimaal MBO-niveau 3;
  • Affiniteit met de doelgroep ouderen. Dit is gewenst, maar niet noodzakelijk;
  • Goede communicatieve vaardigheden, zowel professioneel als binnen de driehoek van zorg;
  • Een flexibele werkhouding t.o.v. dag-, avond-, en weekenddiensten.
