Ons team heeft je hart nodig

Het team maatschappelijk werkenden is op zoek naar een nieuwe collega voor het cluster Voortgezette Behandeling & Verblijf (de langdurige zorg) met ingang van 1 maart 2027. Het team bestaat uit vijf collega’s die ieder hun eigen caseload hebben, maar nauw samenwerken. Je kunt altijd op elkaar terugvallen voor advies, overleg of het delen van informatie.

In jouw werk is geen dag hetzelfde. Je coördineert de integrale procesgang van de psychosociale behandeling en begeleiding rond toegewezen cliënten, waarbij je de eigen regie van cliënten stimuleert. Waar nodig ondersteun of begeleid je of neem je de taken over. Hierin blijf je een coachende rol aannemen om de zelfregie te versterken en het probleemoplossend vermogen te vergroten. De ondersteuning stem je af met behandelaren en de verpleegkundige die belast is met de zorgcoördinatie en organiseert periodiek cliëntbesprekingen. Je schakelt waar nodig met (externe) disciplines. Ook organiseer je bijvoorbeeld familieavonden en ben je het eerste aanspreekpunt voor psycho-educatie.

Als maatschappelijk werker neem je deel aan het MDO (Multi Disciplinair Overleg). Je denkt mee op beleids- en maatschappelijk niveau vanuit het oogpunt van de cliënt. Belangrijk is dat je de psychiatrie en doelgroepen kent, kunt inschatten waar de achterliggende hulpvraag vandaan komt.

  • Je beschikt over een afgeronde HBO opleiding maatschappelijk werk (of vergelijkbaar);
  • Ervaring of affiniteit met de doelgroep is een must;
  • Je bent beschikbaar voor 24 uur per week;
  • Je bent in bezit van een rijbewijs B;
  • Je beschikt over goede coördinerende vaardigheden, bent proactief, flexibel en beschikt over een hoge mate van zelfstandigheid;
  • Jouw observerend vermogen en goede communicatieve vaardigheden zorgen ervoor dat jij onvoorziene situaties de baas kunt zijn;
  • Jij bent een fijne en gezellige aanvulling op ons team!;
  • Om in aanmerking te komen voor deze functie is een positieve Verklaring Omtrent Gedrag (VOG) vereist;
  • Het natrekken van referenties en het controleren van diploma’s maken onderdeel uit van de sollicitatieprocedure.

Hoe we jou voorop zetten

  • Wij bieden je een marktconform salaris CAO GGZ FWG 55. Het salaris bedraagt minimaal € 4.000 en maximaal € 5.294 bij een fulltime dienstverband, afhankelijk van jouw kennis en ervaring;
