Ben jij een verpleegkundig specialist of physician assistant die wil bijdragen aan betere zorg voor kwetsbare ouderen op de SEH en in de kliniek? Dan is deze functie binnen ons GEM-team iets voor jou!

Als verpleegkundig specialist/PA kom je te werken in het Geriatric Emergency Medicine team (GEM-team). Dit is een multidisciplinair team dat zich richt op de zorgbehoeften van kwetsbare ouderen op de SEH en in de kliniek. Het betreft een uitdagende en innovatieve functie binnen ons ziekenhuis, waar je echt impact kan maken! Dit in een inspirerende werkomgeving waarin multidisciplinair samenwerken centraal staat.

In jouw functie ben je verantwoordelijk voor het signaleren en analyseren van de complexe zorgbehoeften van oudere patiënten op de SEH. Je hebt een adviserende rol over diagnostiek, behandelopties en eventueel uitplaatsing naar een passende zorgomgeving.

Daarnaast adviseer en begeleid je de patiënt van het zorgtraject bij opname waarbij je samenwerkt met collega’s in de kliniek met als doel de zorg voor de oudere patiënt naar een hoger niveau te tillen.

Als verpleegkundig specialist/PA word je 1 a 2 dagen per week ma-vrij van 12.00-20.00 uur ingezet in het GEM-team. Daarnaast werk je 1 a 2 dagen per week in het consultenteam van de klinische geriatrie van 7.30/08.00 - 16.00/16.30 uur.

Over het GEM-team

Het Geriatrisch Expertiseteam (GEM-team) is een multidisciplinair team dat zich richt op de zorgbehoeften van kwetsbare ouderen op de SEH. Je krijgt hierbij ondersteuning van de geriater en werkt nauw samen met collega’s uit andere disciplines, zoals SEH-artsen, medisch specialisten en verpleegkundigen om optimale zorg te bieden en complicaties te voorkomen. In dit team verdeel je de dagen met twee andere VS-en.

Jouw vaardigheden:

  • Diploma Master Advanced Nursing Practice (MANP) of Master Physician Assistant.
  • Geldige BIG registratie als verpleegkundig specialist/PA.
  • Ervaring of affiniteit met kwetsbare ouderen en multidisciplinaire samenwerking.
  • Een proactieve houding en het vermogen om snel tot de kern van complexe situaties te komen.
  • Goede communicatieve vaardigheden en het vermogen om helder te adviseren.

Rijnstate biedt jou:

  • Een uitdagende functie binnen een innovatief team op de spoedeisende hulp en geriatrie.
  • Inschaling FWG 60 (max. € 6.220, - op basis van fulltime dienstverband, 36 uur per week.).
  • Een arbeidsovereenkomst van 24 uur voor de duur van 12 maanden, wat bij goed functioneren wordt omgezet in een vast dienstverband.
  • Flexibele werktijden; In overleg met twee collega verpleegkundig specialisten werk je van 12.00 uur tot 20.00 uur op de SEH, de overige contracturen werk je mee als verpleegkundig specialist geriatrie/PA geriatrie in het consultenteam van de klinische geriatrie.
