Als wijkverpleegkundige bij Amarosa kan jij écht het verschil maken in de thuissituatie van cliënten. Je krijgt de ruimte om de regie te voeren over zorg, collega's te coachen en samen te werken in een hecht en betrokken team. Dankzij onze kleinschaligheid zijn de lijnen kort en staat persoonlijke aandacht centraal – voor cliënten én medewerkers.

Wat ga je doen?

Als wijkverpleegkundige bij Amarosa ben jij de regisseur van de zorg thuis. Je indiceert, coördineert en bewaakt de kwaliteit van zorg voor cliënten die zo lang mogelijk zelfstandig thuis willen blijven wonen. Je werkt nauw samen met cliënten, mantelzorgers, huisartsen en collega's en bent een belangrijke schakel binnen het zorgnetwerk.

Dit doe je door:

  • Indicaties te stellen en zorgplannen op te stellen, evalueren en bij te stellen;
  • De regie te voeren over het zorgproces van cliënten in de wijk;
  • Verpleegkundige zorg te verlenen en complexe zorgsituaties te begeleiden;
  • Collega's te coachen en ondersteunen in hun professionele ontwikkeling;
  • Deel te nemen aan regionale overleggen en mee te denken over hoe we de wijkverpleging toekomstbestendig organiseren;
  • Samen te werken met interne en externe zorgpartners om de best mogelijke zorg te realiseren.

Wie ben jij?

Jij bent een betrokken en deskundige professional die zelfstandig werkt en gemakkelijk verbinding maakt met cliënten, mantelzorgers en collega's. Je ziet kansen om zorg te verbeteren en neemt hierin initiatief.

Verder beschik je over:

  • Een diploma HBO-Verpleegkunde en een geldige BIG-registratie;
  • Ervaring in de thuiszorg of wijkverpleging;
  • Kennis van indicatiestelling en zorgcoördinatie;
  • Goede communicatieve en coachende vaardigheden.

  • Een contract tussen 24 en 32 uur voor onbepaalde tijd werkdagen en -tijden in overleg;
  • Een salaris volgens de CAO Verpleeg- en Verzorgingshuizen & Thuiszorg is ingedeeld in FWG 55 (min €3.722,- max. €5.413 bij 36 uur);
  • Vakantietoeslag van 8% en een eindejaarsuitkering van 8,33% over het bruto jaarsalaris;
  • Meerkeuzesysteem arbeidsvoorwaarden (bijvoorbeeld voor de aankoop van een elektrische fiets of het kopen van extra verlofdagen;
  • Reiskostenvergoeding;
  • Een pensioenregeling bij pensioenfonds Zorg en Welzijn;
  • De mogelijkheid om écht het verschil te maken voor cliënten in de wijk.
