Wat krijg je van ons?

Bij Cordaan bieden we jou een uitdagende functie als Persoonlijk Begeleider GGZ in Amsterdam binnen een betekenisvolle organisatie:

  • Contract: Een jaarcontract voor 24–28 uur per week, met uitzicht op een vast dienstverband bij goed functioneren
  • Salaris: Salaris volgens cao GGZ, FWG 45: minimaal €3.180,- en maximaal €4.273,- bruto per maand op basis van 36 uur
  • Extra’s: Eindejaarsuitkering (8,33%) en vakantietoeslag (8,33%)
  • Overig: Pensioenopbouw via Pensioenfonds Zorg & Welzijn Reiskostenvergoeding conform CAO
  • Fietsplan en fietsreparatievergoeding
  • Ontwikkelmogelijkheden: Volop mogelijkheden om je persoonlijk en professioneel te ontwikkelen
  • Collega aandragen? Ontvang in totaal een aanbrengbonus van € 500,- netto

Waar ga je werken?

Welkom op de Van Ossstraat, een kleinschalige woonlocatie in Amsterdam waar 24 cliënten met een psychische kwetsbaarheid zelfstandig wonen in hun eigen appartement.

Wij bieden een 16-uurs voorziening en begeleiden daarnaast ook een aantal cliënten ambulant in de wijk. De cliënten zijn overwegend stabiel, maar er kan sprake zijn van crisisgevoeligheid. We werken nauw samen met het F-ACT team GGZ en crisisondersteuning waar nodig.

De focus ligt op:

  • Herstelgericht werken
  • Vergroten van zelfstandigheid en zelfredzaamheid
  • Ondersteunen zonder over te nemen
  • Stimuleren van dagbesteding, werk en sociale contacten

Het team bestaat uit 7 collega’s (persoonlijk begeleiders, een assistent-begeleider en een coördinerend persoonlijk begeleider). Het is een open, betrokken en gezellig team met veel jonge professionals. Daarom zoeken we een ervaren collega (minimaal 3 jaar GGZ-ervaring) die stabiliteit, kennis en structuur toevoegt.

Wat ga je doen?

Als Persoonlijk Begeleider GGZ op de Van Ossstraat in Amsterdam combineer je werken op locatie met ambulante begeleiding in de wijk. Je hebt een eigen caseload van ongeveer 6 cliënten en voert regie over hun begeleidingstraject.

Jouw taken en verantwoordelijkheden:

  • Begeleiden van cliënten bij hun herstelproces en dagelijks functioneren
  • Opstellen, evalueren en bijstellen van zorgplannen
  • Coördineren van zorg en deelnemen aan multidisciplinair overleg
  • Ondersteunen bij wonen, financiën, sociale contacten, werk en dagbesteding
  • Signaleren van veranderingen in psychisch functioneren en hier adequaat op handelen
  • Meedraaien in dag-, avond- en weekenddiensten (ongeveer 2 weekenden per maand)
  • Vervullen van neventaken, zoals aandachtsfunctionaris of medicatiebeheer
  • Je werkt herstelgericht, stelt duidelijke grenzen waar nodig en weet goed balans te houden tussen nabijheid en professionele afstand.

Wie ben jij?

  • Je hebt een relevante hbo-opleiding afgerond (bijvoorbeeld Social Work of SPH) óf een mbo 4-diploma zorgdiploma. Met een mbo-4-zorgdiploma wordt het hbo-niveau beoordeeld aan de hand van een assessment.
  • Je hebt minimaal 2 jaar werkervaring binnen de GGZ
  • Ervaring met complexe psychiatrische problematiek is een must
  • Je hebt ervaring met zorgplannen, coördinatie van zorg en multidisciplinair samenwerken

Je bent:

  • Stevig en duidelijk in je communicatie
  • Reflectief en professioneel
  • Flexibel inzetbaar (dag, avond en weekend)
  • Iemand die humor meebrengt en energie haalt uit teamontwikkeling
