Als Bewegingsagoog bij Inovum breng jij cliënten letterlijk en figuurlijk in beweging. Je organiseert en begeleidt individuele én groepsgerichte bewegingsactiviteiten op meerdere locaties, afgestemd op de doelgroepen Psychogeriatrie, Somatiek en NAH. Jouw doel? Het verbeteren van het bewegingsgedrag, de motoriek én het psychosociaal functioneren van onze cliënten. Je werkt nauw samen met het multidisciplinaire behandelteam en stelt op basis van intake en observatie een persoonlijk bewegingsactiviteitenplan op. Tijdens de uitvoering observeer je het functioneren van cliënten, geef je instructies, motiveer je hen en stuur je bij waar nodig. Je rapporteert veranderingen en voortgang in het digitale zorgdossier en denkt actief mee over nieuwe bewegingsvormen en technieken. Jouw verantwoordelijkheden: • Organiseren en uitvoeren van bewegingsactiviteiten op maat, individueel en in groepsverband. • Opstellen en bijstellen van bewegingsactiviteitenplannen in overleg met het behandelteam. • Signaleren van veranderingen in het functioneren van cliënten en rapporteren in het ECD. • Implementeren van nieuwe bewegingsvormen en technieken binnen het aanbod. • Beheren van materialen, werkruimtes en voorraad Je werkt zelfstandig, maar bent altijd verbonden met je collega's binnen de behandeldienst. Samen zorgen jullie voor een passend en stimulerend aanbod voor elke cliënt. Wie ben jij? Jij bent een enthousiaste en betrokken professional die gelooft in de kracht van beweging. Je weet cliënten te motiveren, te stimuleren en te begeleiden - ook als het even lastig is. Je hebt oog voor veiligheid, groepsdynamiek en het welzijn van de cliënt.
